Lipanthyl Supra — co to za lek?

Lipanthyl Supra (160 mg / 215 mg) is a drug indicated for use as an addition to the diet and other non-pharmacological therapies (e.g. exercise, weight loss) in the following cases:

  1. treatment of severe hypertriglyceridaemia with or without low HDL cholesterol
  2. mixed hyperlipidaemia when statin use is contraindicated or not tolerated,
  3. mixed hyperlipidaemia in people at high risk of developing cardiovascular disease, in addition to statin therapy, when triglycerides and high-density cholesterol (HDL) are not adequately controlled.

The active substance of the preparation Lipanthyl Supra is fenofibrate. It belongs to a group of medicines called fibrates that are used to lower the levels of lipids (cholesterol, triglycerides) in the blood.

Lipanthyl Supra – mechanism of action

Fenofibrate, the active substance. Lipanthyl Supra is a derivative of fibric acid, the lipid modifying effect of which in humans is achieved through the activation of nuclear receptors of the α-type (PPARα, Peroxisome Proliferator Activated Receptor type α).

By activating PPARα, fenofibrate increases lipolysis and the elimination of serum triglyceride-rich atherogenic particles by activating lipoprotein lipase and reducing the production of apolipoprotein CIII.

Activation of PPARα also leads to an increase in the synthesis of apolipoproteins AI and AII. The effect of fenofibrate on lipoproteins leads to a decrease in the very low and low density fractions (VLDL and LDL) containing apolipoprotein B and an increase in the high density lipoprotein (HDL) fraction containing apolipoproteins AI and AII.

Fenofibrate is mainly excreted in the urine. It is completely eliminated within 6 days. Fenofibrate is mainly excreted in the form of fenofibric acid and its glucuronide derivatives.

Lipanthyl Supra – dosage

Always take Lipanthyl Supra exactly as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure. Your doctor will determine the correct dose of the drug depending on your medical condition.

A Lipanthyl Supra tablet should be swallowed with a glass of water. The preparation should be taken with a meal, as the absorption of the drug on an empty stomach is much worse.

Dosing of Lipanthyl Supra is as follows.

Adults

  1. the recommended dose is 1 160 mg / 215 mg film-coated tablet daily.
  2. people who are currently taking capsules containing 200 mg of fenofibrate (1 capsule a day) can start taking 1 tablet of 160 mg a day without dose adjustment.

People with renal insufficiency

In people with renal insufficiency, the doctor may reduce the dose. In the event of such disturbances, consult your doctor or pharmacist. In people with severe renal insufficiency (creatinine clearance <20 ml / min), the drug is contraindicated.

Elderly people

For elderly patients without renal insufficiency, the recommended adult dose is.

People with liver failure

Lipanthyl Supra is not recommended due to the lack of clinical data in people with hepatic insufficiency.

Use in children and adolescents

The use of Lipantil Supra in children and adolescents under 18 years of age is not recommended.

Lipanthyl Supra – contraindications

The main contraindication to the use of Lipanthyl Supra is hypersensitivity to the active substance of the drug or auxiliary substances. In addition, Lipanthyl Supra is not recommended for:

  1. liver failure (including biliary cirrhosis and unexplained prolonged liver dysfunction),
  2. gallbladder disease,
  3. severe renal failure (eGRF <30 ml / min / 1,73 m2),
  4. chronic or acute pancreatitis with the exception of acute pancreatitis due to severe hypertriglyceridaemia,
  5. photosensitivity or phototoxic reactions when using fibrates or ketoprofen.

Pregnant and breastfeeding women should consult a doctor before using Lipanthyl Supra. In general, you should not take this preparation during pregnancy and breastfeeding.

Lipanthyl Supra should not be used in people who are hypersensitive to peanuts, peanut oil, soy lecithin or derivatives due to the risk of hypersensitivity reactions.

Lipanthyl Supra – precautions

Talk to your doctor or pharmacist before taking Lipanthyl Supra 160 if:

  1. have liver or kidney problems
  2. have inflammation of the liver, symptoms include yellowing of the skin and whites of the eyes (jaundice) and increased levels of liver enzymes (shown in laboratory tests)
  3. you have an underactive thyroid gland (decreased activity of the thyroid gland).

If any of the above warnings applies to you (or if you are in doubt), check with your doctor or pharmacist before taking Lipanthyl Supra.

Lipanthyl Supra – effect on the muscles

While taking Lipanthyl Supra you may experience unexpected muscle cramps or pain, muscle tenderness or weakness while taking this medicine. Lipanthyl Supra can cause muscle problems which can be severe. These conditions are rare but include muscle inflammation and breakdown. This can cause kidney damage or even death.

Your doctor may do blood tests to check the condition of your muscles before and after starting treatment. The risk of muscle breakdown may be greater in some patients. Please inform your doctor if:

  1. the patient is over 70 years old,
  2. have kidney disease
  3. have thyroid disease
  4. you or someone in your family have a hereditary muscle disease
  5. a sick person drinks large amounts of alcohol,
  6. you are taking medicines to lower your cholesterol levels called statins, such as simvastatin, atorvastatin, pravastatin, rosuvastatin or fluvastatin.
  7. a history of muscle problems while taking statins or fibrates such as fenofibrate, bezafibrate or gemfibrozil.

Lipanthyl Supra – interactions with other drugs

Before taking Lipanthyl Supra, tell your doctor or pharmacist if you are taking medicines such as:

  1. anticoagulants taken to thin the blood (e.g. warfarin)
  2. other medicines used to control blood fat levels (such as statins or fibrates). Taking a statin at the same time as Lipanthyl Supra may increase the risk of muscle damage.
  3. medicines from the group of medicines used to treat diabetes (such as rosiglitazone or pioglitazone) – cyclosporine (an immunosuppressive medicine). 

Lipanthyl Supra – possible side effects

The most commonly reported side effects with fenofibrate are digestive, gastric or intestinal disorders.

Common side effects (may affect up to 1 in 10 people):

  1. diarrhea,
  2. stomach pain,
  3. flatulence with wind,
  4. nausea,
  5. vomiting,
  6. increased levels of liver enzymes in the blood
  7. increased levels of homocysteine ​​in the blood.

Uncommon side effects (may affect up to 1 in 10 people):

  1. headaches,
  2. cholelithiasis,
  3. decreased sex drive,
  4. rash, itching or hives
  5. increase in creatinine excreted by the kidneys.
