Anterior rhinoscopyThe doctor fixes the patient’s head with his right hand, placing it on the back of his head, and with his left hand he inserts the closed branches of the nasal speculum (nasal dilator) into the vestibule of the nasal cavity, opening them there. The mucous membrane of the nasal cavity, the anterior and part of the middle part of the nasal septum, the anterior parts of the inferior and middle nasal concha, and the bottom of the nasal cavity are examined.

Rhinoscopy (examination of the nasal cavity) is performed using a frontal reflector using nasal dilators (mirrors), which allow you to expand and lift the nostrils and thus make the nasal cavity accessible for illumination and inspection.

Anterior rhinoscopy performed with the head in normal position. By spreading the nostrils with a nasal speculum, they examine the lower floors of the nasal cavity – the lower nasal passage. In this case, the anterior end of the inferior concha and the anterior sections of the nasal septum are visible. If you tilt your head back a little, you can examine the middle and upper sections of the nasal cavity. In this case, the anterior end of the middle concha is visible, and sometimes, with sufficient width of the nasal passages, it is also possible to examine the posterior sections of the concha and nasal septum and the posterior wall of the nasopharynx.

Posterior rhinoscopy (examination of the posterior parts of the nose and nasopharynx) is performed through the mouth; They use small mirrors with a diameter of 8-10 mm. The angle between the mirror and the rod (handle) should be 115° (and not 125°, as with laryngoscopy mirrors).

Rhinoscopy technique

First, slightly heat the mirror over an alcohol lamp. The patient’s tongue is slightly pressed downwards with a spatula, the mirror is inserted upward with the glass and installed in the depths of the pharynx under the uvula and, if possible, behind it and the soft palate. The patient is asked to relax the soft palate, which is facilitated by maximum simultaneous inhalation of air through the nose. Posterior rhinoscopy allows you to examine the nasopharyngeal space.
