Monika Mikołajska / Medonet: We know that pregnancy increases the risk of thrombosis. How does this relate to the information on cases of this type of complications after the administration of AstraZeneki and Johnson & Johnson vaccines.

Dr. Jacek Tulimowski, obstetrician gynecologist, editor-in-chief of the magazine «New Gynecological Cabinet: We have known for a long time that the risk of thrombosis increases in pregnancy. Remember, however, that the risk is somehow inscribed in medicine – just like everywhere in the world, here too – we cannot give 100 percent. warranty. Meanwhile, at present, hedonistic or even selfish attitudes prevail. 20 years ago, a patient came to me asking to plan a pregnancy, today she asks me if I can guarantee her that she will give birth to a healthy child.

Let’s move on. It is known that some women use contraception for some time before becoming pregnant. How many of them had clotting monitoring? They are carried out before, during and after contraception. I leave this question open. Does a woman need any blood tests to be vaccinated against COVID-19? There are no findings. So how is she to know if she has, for example, a bleeding disorder, thrombophilia or other similar problems? The woman is then vaccinated against COVID-19, and if she develops an embolism, she will likely think it is because of the vaccine. However, there is no evidence for this.

Another important point, the European Medicines Agency has reiterated its assessment that the benefits of vaccinating with AstraZeneka outweigh the risks of very rare and unusual side effects. In the case of the Johnson & Johnson vaccine, the EMA also concluded that the benefits outweigh the risks many times over.

The conclusion is that before vaccinating against COVID-19, a pregnant woman should make an appointment with her doctor.

Yes. Every pregnant woman in Poland has her own attending physician. So if she has any doubts about getting vaccinated against COVID-19, she should come to him and ask. Based on the information from the documentation, the gynecologist will suggest a solution. At the same time, he is obliged to explain all the pros and cons to her and, importantly, he cannot dissuade her from wanting to get vaccinated.

The possibility of vaccination against COVID-19 should be considered individually for each pregnant woman. During the conversation with the patient, the doctor should assess the benefits against the risks as precisely as possible and explain everything to her. My patients very often ask me for my opinion whether they are to be vaccinated or not, or if they can wait. I guarantee that I will have more and more questions.
