Urinary incontinence and frequent nocturnal urination (nocturia) in older men are associated with an increased risk of death, even if “they are not troublesome,” reports The Journal of Urology.

The new research was carried out by Dr. Jonne Akerl’s team from Tampere University in Finland. The data used came from long-term health follow-up studies of over 3 people. Finnish men who were 1994, 50 or 60 years old at the time of study enrollment in 70.

The analysis (DOI: 10.1097 / JU.0000000000002450) included data on 1167 men who were followed up to the end of 2018. About half of the subjects died during this 24-year follow-up period.

Some associations with mortality have been reported even when urinary symptoms were not very troublesome for the patient and may not be considered requiring treatment. This demonstrates the importance of assessing overall health, risk factors and major comorbidities, the authors point out.

Lower urinary tract complaints (LUTS) were analyzed as a risk factor for death during follow-up, taking into account age and other comorbidities (comorbidities). The analysis included a possible relationship between symptoms and nuisance. Urologists believe that men who say their urinary symptoms aren’t bothering them much are successful in managing their symptoms and therefore don’t require treatment.

The analysis showed significant associations between male urinary symptoms and long-term risk of death. The risk was 20 percent higher. for men with moderate to severe bladder emptying symptoms, such as trouble starting to urinate, a weak urine stream, or the urge to urinate hard. There was also 40 percent. increased risk in men with “storage” symptoms such as frequent daytime urination (voiding), nocturia or various types of urinary incontinence.

Mild urinary symptoms showed no increased risk of death. “Moderate and severe LUTS in men are generally indicators of poor health, while mild LUTS can be considered a normal part of healthy aging,” the authors point out.

There were, however, some notable exceptions: the risk of death increased by 30%. among men with frequent urination during the day and 50%. in the case of nocturia, regardless of the severity of symptoms. Dr. Akerla and colleagues write: “This suggests considering a daytime interval of three hours or less, and any nocturnal urination as important prognostic factors for the patient, especially when they appear as persistent symptoms.”

In addition, the risk of death was more than twice as high among men with frequent urinary incontinence. This “particularly strong” association suggested that urgency to urinate has a significant impact on the health and functional status of aging men – possibly reflecting the effects of long-term neurological disorders and blood vessel disease (PAP).
