Botox copes well with mimic wrinkles. However, the skin around the eyes is delicate and causes many other problems. How to get rid of drooping eyelids, shadows and so-called bags under the eyes? Joanna Kuschill-Dziurda, MD, PhD, medical director of the Medestetis Institute of Aesthetic Medicine, tells us.

- Collagen – action, impact on health, sources. Which collagen to choose?
Collagen is an essential building block of tissues. Thanks to it, the skin is firm and smooth. However, the older we get, the less the body produces. This makes for …

- Chiropractic
Chiropractic is a method of manual therapy whose roots go back to antiquity. It is one of the areas included in the field of chiropractic. He treats …
They arise as a result of the action of the facial muscles. The mimic muscles are very strongly attached to the skin, which makes their every movement visible in the form of wrinkles. Botulinum toxin, commonly known as botox, copes well with the problem. – Given in the form of intramuscular injections, it is also used in medicine to treat excessive sweating, bruxism and migraines. It inhibits the release of acetylcholine, a neurotransmitter responsible for transmitting signals between neurons and muscle cells. It works in two ways: it reduces muscle contractility (the skin relaxes and wrinkles disappear) and prophylactically (the skin rests from constant movement, which improves its condition after the botox effect ceases) – explains Joanna Kuschill-Dziurda, MD, PhD. The treatment is safe and reversible – the effect lasts on average from three to six months. Attention! The injection should be performed by an experienced physician. Incorrectly selected or inadequately administered dose poses a risk of the “mask” effect or side effects.
The skin under the eyes is very thin – thinner than in other parts of the face, which makes it ages the fastest. A visible mesh of fine wrinkles appears. Their cause is, among others weakening of collagen and elastin fibers. Laser therapy, mesotherapy, chemical peeling, platelet-rich plasma, PDO threads, or plasma treatment may be helpful – explains Dr. Joanna Kuschill-Dziurda. The purpose of these treatments is to stimulate the production of collagen and elastin as a result of activating the skin healing processes. The spectrum of treatments is very wide and you have to take into account the patient’s age, expectations and the possible need to withdraw from professional and social activity for several days. A frequently performed procedure is mesotherapy around the eyes, often with the use of platelet-rich plasma. The advantage of mesotherapy is its comprehensive action: biostimulation, hydration, firming and reconstruction of the skin. Laser or plasma treatments usually require switching off for a few days, but they give spectacular effects of skin remodeling. Biorevitalizing PDO threads are also used to strengthen the skin around the eyes and chemical peels are made for this area. Chemical peeling gives a comprehensive effect – it reduces discoloration, fine wrinkles, and mobilizes the skin to rebuild. With age, the regenerative abilities of the skin, unfortunately, slow down and non-invasive treatments do not give such a spectacular effect. Traditional plastic surgery will work better then.
The cause of dark circles under the eyes is sometimes excessive accumulation of haemosiderin in the skin of the eye sockets, which causes a change in skin color. Then, the best treatment is special peels that brighten this area or mesotherapy that brightens the eye area. Mesotherapy, i.e. small skin punctures with the use of special brightening substances in the form of mesococtails. The downside is the appearance of several hours of swelling under the eyes or minor bruises. Another cause of dark circles under the eyes may be the lack of fatty lining, which causes the skin to lie directly on the bone and circular muscle of the eye, which optically changes its color. In this case, filling with hyaluronic acid will work best. – It is relatively safe and has an immediate effect. The procedure is performed with the use of a microcannula and no convalescence is required after it – says Dr. Joanna Kuschill-Dziurda. An option is to transplant fat from a different area of the body, but it requires several days of convalescence and a larger surgery. Many people are concerned that if they undergo aesthetic medicine treatments before the age of 20, their skin will “become lazy”. It’s a myth. Remember, prevention is easier than cure. – If a patient does not care for the condition of the skin until his XNUMXth birthday, and then expects to “turn back” the time by XNUMX years, it is not feasible. Taking care of your skin is like a sport – if you practice it regularly, it will keep your muscles and figure in shape for years. Similarly well-chosen, relatively regularly repeated treatments will keep the good condition and appearance of the skin for a long time – says Dr. Joanna Kuschill-Dziurda from Medestetis.
«Bags» under the eyes Most often occur as a result of a simultaneous loss of elasticity, loss of fatty lining and weakening of collagen fibers. Combined treatments will work here, and if we decide on one, the key is to find the root cause of the “bags”. When it is a fatty hernia (usually in the elderly), blepharoplasty will be the best – removing excess skin from the lower eyelid along with the fat cells accumulating under it. This is a radical, but one-off procedure (the effect is permanent). The cut can be done in two ways: 2 mm under the eyelashes or from the conjunctival side of the eyelid. What if we want to avoid the scalpel at all costs? The solution is plasma. A treatment that uses electrical impulses, locally lifting the skin, without the risk of damaging the dermis, bleeding, burns and inflammatory reactions in the surrounding tissues. After the treatment, point scabs are formed (they usually last up to a week) and the eyelids may be swollen for 2 days. The procedure is performed under local anesthesia (cream with lidocaine).
Pigmentation discoloration We risk discoloration around the eyes by forgetting to wear sunglasses and use sunscreen. We can start the fight with discoloration only in autumn and winter. A chemical peel will work, lighten discoloration, and at the same time rebuild the skin and improve its tension. After peeling, the skin exfoliates for about a week, and for at least a month we have to avoid the sun to avoid discoloration. A very good solution is the use of plasma – a new technology that allows the pigmentation change to evaporate. After the treatment, the skin heals for about 4 weeks, but after a few days you can apply makeup. You have to remember that not every device can be operated around the eyes, explains Dr. Joanna Kuschill-Dziurda from Medestetis.
If you are afraid of a scalpel, you can opt for a non-surgical eyelid lift. The aforementioned Plasma treatment is suitable for this (safe and controlled evaporation of the epidermis will lead to smoothing and increased skin tension, thus giving a lifting effect). Laser treatments (a strong regenerative effect, also at the level of the dermis) will also work for the rejuvenation of the skin around the eyes. They reduce discoloration, fine wrinkles and scars, without the risk of bruising. – The lifting effect is greatest in the case of thin skin without fat lining. With a thick, so-called heavy skin and a large excess of it, neither laser, fillings or threads will work – they will not be able to lift the skin sufficiently – explains Dr. Joanna Kuschill-Dziurda. Only blepharoplasty – excision of excess skin along with fatty hernias – can handle this. The procedure is performed under local anesthesia. Stitches are left on the skin for a week, hidden in the crease of the eyelids, and the wounds are covered with thin slices. The scars after the procedure will remain invisible.
It is worth considering the eyebrow lifting procedure when they are set so low that they “fall” over the eye or in the case of eyebrow asymmetry. – The most common causes of this type of problem are: collapse of the temples due to the loss of the fatty lining, weakening of the frontal muscle, stretching of the collagen fibers (age-related) or too strong activity of the frowning muscles – says Dr. Joanna Kuschill-Dziurda from Medestetis. Depending on the dominant cause, the doctor selects the correction method. These can be, for example, filling the temples with hyaluronic acid, putting on hook threads to raise the eyebrows or regulating muscle tension by injecting botulinum toxin.
