Hello, I am writing to you with a serious problem related to choosing a lactose-free formula. My daughter developed symptoms of lactose intolerance, which were confirmed by tests. Of course, we immediately changed the served milk to modified milk without lactoseHowever, I am still concerned about whether we made the right choice, taking into account all the needs of the child. Which lactose-free formula is best? I do not care about recommending specific brands, but rather advice on what to look for when choosing lactose-free formula.
I would also like to know if this lactose intolerance that occurs in an infant can reverse later in life? I am a bit worried about it, because if now we simply give lactose-free modified milk, then when expanding the diet, other ones will appear problems with lactose products. Food intolerances, including lactose intolerance, are most often caused by secondary (i.e. due to another disease entity) deficiencies in the activity of digestive enzymes (in this case the lactase enzyme) as a result of a decrease in the lymphatic area of the intestine (e.g. acute diarrhea, post-antibiotic diarrhea, untreated celiac disease or food allergy) ). It is also possible to develop hypolactasia (lactase enzyme is present, just in smaller amounts) of the adult type, genetically determined, inherited autosomal recessively, it occurs in 18-37% of children over 5 years of age.
If we are dealing with intolerance, not an allergy, i.e. probably with what concerns a child, lactose-free or low-lactose milk replacers are used in children under 2 years of age, fermented milk is recommended for older children and adults. You can also give the enzyme lactase together with the milk you drink (you can ask your doctor, Pediatrician).
The prognosis in most children is good, the symptoms of intolerance resolve spontaneously by 4-5 years of age, but sometimes they persist into adulthood. Up to this point, it has not been established how to speed up the recovery from intolerance, so there are no measures that can be implemented to end the intolerance sooner.
The purpose of nutrition in lactose intolerance is avoiding symptoms while providing adequate amounts of calcium – this is the most important parameter, along with protein content, which should be taken into account when choosing lactose-free milk. The daily requirement for calcium as well as vitamin D varies with age, so please ask your doctor for specific recommendations.
Most lactose intolerant patients can tolerate up to 12g of lactose daily without symptoms. A highly restrictive diet should be introduced first, but lactose should be gradually reintroduced into the diet until symptoms reappear. You may find it useful later to eat milk products that have been fermented or matured, and to consume lactose-containing products with other food products.
