What are the indications for the SHBG level test?

Good day. My name is Kasia, I’m 30 years old. I urgently need help. I recently started working in a new company in a management position. Daily activities, although they give me a lot of satisfaction, are also extremely stressful. Sometimes, after returning home, I sometimes eat stress with sweets, but still mentally I feel okay. However, the worst thing about it all is that my hair has started to fall out, and at an alarming rate.

Sometimes, after waking up, there are a lot of them on the pillow. I must admit that it is a terrifying sight for me, because I have always had thick, curly hair. But that’s not all. I have also had menstrual disorders for 1,5 months. Sex is also not as fun for me as it was before and I feel that I am also less able in bed. A friend told me to perform SHGB level test. I have read a bit about it and it makes sense, but I am asking for more information on when to do it and what are the SHGB testing standards and do I qualify for them at all? The doctor explains when the SHBG test should be performed

SHBG, that is sex hormone binding globulin it is a protein, more specifically a sex steroid binding globulin. It is produced in women mainly by the liver and binds most of the circulating testosterone in the blood, and the amount of this protein indirectly regulates the activity of circulating androgens. If there is a reduction SHBG synthesis (e.g. by hyperinsulinemia in polycystic ovary syndrome) this will increase the pool of free androgens and cause symptoms of hyperandrogenization. The symptoms of hyperandrogenization include: hirsutism, i.e. the appearance of male hair type, seborrhoeic changes, acne, oily hair. There may also be alopecia, but then it has the character of “male” baldness.

The symptoms you describe do not necessarily indicate the need for SHBG testing, in your case stress-induced hyperprolactinemia seems more likely. The main symptoms of hyperprolactinaemia are: menstrual disorders, ovulation disorders, decreased libido and even pain during intercourse (dyspareunia), headaches and visual disturbances, decreased bone mineral density.

However, to confirm or rule out any of the diagnoses, you must see a gynecologist. Only after collecting a detailed history of the symptoms, previous medical history, medications taken and physical examination, using a vaginal scan, two-handed examination and transvaginal ultrasound, decide what tests to order and what treatment to prescribe.

You should also consider whether the stress you encounter at work is manageable. Maybe it is worth talking to a specialist or finding ways to relax after work or during work? Stress can have many negative effects on health, it is worth fighting so that chronic stress does not take over our lives.
