Anyone who decides to vaccinate against COVID-19 receives a special form at the vaccination point. The document contains basic personal data and the date of receipt of the preparation, as well as the batch number of the drug. But what if we lose this little piece of paper?
- The card from the vaccination point contains personal data, dates of administration of doses of the preparation, as well as vaccine batch numbers
- If the card is lost, you can download the vaccination certificate or the EU COVID Certificate from the Patient Online Account.

Card with information about vaccination against COVID-19
The vaccination card may get lost or damaged. Then what? The Ministry of Health explains what to do in such a situation.
– Log in to your Internet Patient Account and download the EU COVID Certificate from the Certificates tab or download the vaccination certificate from the Prevention / Vaccinations tab – explains Justyna Maletka from the MZ Communication Office.
Don’t worry – people who have problems using the Patient Online Account can obtain a certificate in a different way. How?
– You can ask for a printout of both documents from a health care clinic or a vaccination center – explains the Ministry of Health.
EU COVID Certificate
Poland joined the EU COVID Certificate program, which means that we will only need this one document.
Justyna Maletka from the Ministry of Health explains that The EU COVID Certificate is a document mutually recognized in the EU containing a QR code and a unique identifier, facilitating the verification of the traveler in the EU in terms of epidemic risk.
– From June 1, the certificate in Poland can be downloaded free of charge from the website of the Patient’s Online Account in PDF format suitable for presentation on mobile devices and for printing (login via the website patient.gov.pl). After June 25, UCC will also be available in the mojeIKP and mObywat applications, he says.
The certificate can also be obtained in the form of a hard copy at the vaccination center.
Healers or people with a negative coronavirus test result will be able to receive a UCC in the form of a paper printout from any medical worker who has access to the salon.gov.pl application, i.e. from a doctor or nurse from a primary health care clinic.
In Poland, the certificate will be valid for 14 days from the administration of the last dose of the vaccine. This is the time needed for immunity to develop after a complete vaccination course.
