Hello, during the last visit to the endocrinologist, I got indications for basic tests, which I already knew quite well. The only thing that surprised me was something I’d never heard of, namely 3rd generation TSH. I’ve never had this test done before, so I don’t know what the 3rd generation TSG is? Is the 3rd generation TSH different from the regular TSH test? If so, why did my doctor ask me to do this test now? What can you detect by executing 3rd generation TSH test?
I marked it a few years ago Thyroid hormones at an endocrinologist and no one even mentioned that there are different types of TSH tests. Now, I do not know if it is good that the doctor ordered me TSH of the 3rd generation? Because I assume that since it is newer, it should detect more diseases or hormonal disorders than previous TSH tests. I will be grateful for dispelling my doubts, because I have no idea what they look like standards for the 3rd generation TSH and whether they are different from the usual. Regards

Generation 3 TSH is currently a test routinely used in Polish laboratories and there is no obligation to mark it to be performed with this method, because this test will certainly be performed. Most likely, you have also examined you earlier TSH – the 3rd generation testso there is no reason to expect new, different results than before.
The reference values vary depending on the laboratory, however, it is assumed that the standard is between 0.4-4.0 mIU / l. In hypothyroidism, the concentration of the hormone is increased, and in hyperthyroidism, it is lowered.
In addition to testing TSH, the doctor can also measure the level of fT3 and fT4, while their elevated values may indicate hyperthyroidism, and decreased values - hypothyroidism.
Hypothyroidism may be subclinical, ie asymptomatic, and it is diagnosed when the levels of thyroid hormones, i.e. fT3 and fT4, are normal and TSH levels are lowered. Conversely, overt hypothyroidism is diagnosed when TSH is elevated and free thyroid hormones are lowered. Manifest hyperthyroidism is an absolute indication for treatment, but it is subclinical in selected cases.
TSH is used to control thyroid disease because of its greater sensitivity and the fact that it reacts faster to an endocrine disruption than free hormones.
