Back and neck pain is a common companion of a person leading a sedentary lifestyle. This problem lies in wait for pupils and students, office workers and people of creative professions – everyone who spends long hours at a computer or desk. Muscle pain accompanies many diseases of the joints and spine, causes significant discomfort and reduces the quality of life.

In the treatment of muscle pain, a combination of NSAIDs with muscle relaxants is used. In this article, we will discuss only the last group of drugs and pay attention to two drugs for muscle pain – Tolperisone and Mydocalm. How they work and how they differ – read on.

Original and generic: which remedy can be trusted?

Mydocalm is an original Hungarian-made drug. It has proven itself in clinical trials and has been recognized as a fairly effective and safe agent:

  1. The largest study of Mydocalm was conducted in 2013. It was attended by 2090 medical institutions from 13 countries – more than 35 thousand patients in total. The high efficiency of the drug in the treatment of pain in muscle spasm was demonstrated. In 19% of cases, adverse reactions were registered, most of which (84,48%) were mild.

  2. In a randomized, double-blind, placebo-controlled study, provedthat Mydocalm, prescribed for stroke, really relieves pain and restores muscle tone – and therefore speeds up rehabilitation.

  3. In the clinical study “Sail” it was shownthat Mydocalm relieves pain and relaxes muscles by the third day of treatment. The maximum effect was seen on the 10th day of therapy. During the trial, it was also proved that the drug does not affect blood pressure and does not have a sedative effect, which allows it to be used for therapy in various groups of patients.

  4. Tolperisone is a generic, that is, an analogue of the original Mydocalm. There are not many studies on the Russian drug, but one of them deserves attention. In 2018 there were provedthat Tolperisone in combination with NSAIDs well relieves muscle spasm and relieves pain – and acts much more effectively than with the isolated appointment of anti-inflammatory drugs. But whether the Russian Tolperisone will be as effective as the Hungarian Mydocalm remains unclear.

How muscle relaxants relieve pain in mice

Mydocalm and Tolperisone contain one active ingredient – tolperisone hydrochloride. It is a centrally acting muscle relaxant – it relaxes the muscles by acting through the structures of the brain.

It is still not known exactly how muscle relaxants act on the human body. In pharmacological practice, it is customary to divide them into two groups:

  1. Central action drugs. They affect motor neurons located in the tissues of the brain and spinal cord.

  2. Peripheral drugs. They work in neuromuscular synapses – where nerve fibers and muscle endings connect. Do not affect the brain and spinal cord.

Tolperisone hydrochloride acts as follows:

  1. Stabilizes the membranes of nerve cells – reduces their permeability.

  2. Blocks the channels that let sodium into the cell, and also interferes with the flow of calcium.

  3. It prevents the conduction of nerve impulses along the fibers and reflex arcs.

All this interferes with the passage of impulses through neurons – a person simply stops feeling pain. Muscles relax – this is how muscle relaxants allow you to break the vicious circle of pain – spasm – pain. It is not enough just to relieve pain with NSAIDs (for example, ibuprofen or diclofenac tablets), you also need to relieve muscle spasm. This is the only way to achieve a good effect and quickly alleviate the condition.

Tolperisone hydrochloride has another effect – it increases blood flow in muscle tissue. Relaxed muscles receive more oxygen and gradually recover – which means that the risk of relapse is reduced. It is important to note: the increase in blood flow occurs due to the mild vasodilating effect of the drug. At the same time, tolperisone does not affect systemic blood flow and blood pressure.

Indications for use

Mydocalm and Tolperisone have the same composition, so they will be used in the same situations:

  1. Diseases of the joints, spine and muscles, leading to the appearance of muscle pain and spasm: arthrosis of large joints, osteochondrosis, spondylosis, cervical and lumbar syndromes.

  2. Organic lesions of the central nervous system: complex therapy for stroke, multiple sclerosis, etc.

  3. Encephalopathies accompanied by muscle dysfunction: Little’s disease, etc.

Muscle relaxants are extremely rarely used as the only means – in this capacity they are ineffective. Usually, tolperisone hydrochloride is prescribed in complex treatment together with NSAIDs and other drugs.

Application Scheme

Mydocalm and Tolperisone are available in tablets and injections. Usually, in the acute stage, the drug is administered intramuscularly in a short course – up to 3-5 days, then the patient is transferred to tablets. This scheme allows you to quickly stop the pain and relieve muscle spasm, improve the quality of life.

Tablets should be taken orally and only after meals, without chewing, with a glass of water. When taken on an empty stomach, the bioavailability of the drug decreases, and the effectiveness decreases.

The dosage of the drug is selected by the doctor individually, taking into account the severity of the disease and the age of the patient.

Mydocalm, according to the instructions, is allowed for children from 3 years old. Tolperisone is prescribed only for adults over 18 years of age.

Adverse Reactions

The use of Mydocalm and Tolperisone can lead to unwanted symptoms. According to the observations of doctors, most often there are adverse reactions from the digestive tract – abdominal pain, nausea, less often vomiting, diarrhea or constipation.

Rarely appear:

  1. shortness of breath, rapid breathing;

  2. cardiopalmus;

  3. lowering blood pressure;

  4. muscle weakness, muscle pain;

  5. anemia;

  6. allergic reactions, extremely rarely – anaphylactic shock;

  7. anorexia;

  8. sleep disturbances, decreased mood up to depression;

  9. decreased vision, tinnitus;

  10. asthenia, fatigue.

The risk of developing undesirable effects increases with increasing dosage and duration of taking the drug.

Clinical researches showthat against the background of the use of muscle relaxants, adverse reactions occur quite often: up to 84% of patients complain of the appearance of certain symptoms. As a rule, these are mild undesirable effects that do not cause significant discomfort and do not reduce the quality of life. In one of the last Research it has been suggested that the original Mydocalm is less likely to cause adverse reactions than the generic Tolperisone – but so far this issue has not been fully studied.

The instructions for use of the original drug states that Mydocalm can be used when driving a car – it does not affect the speed of psychomotor reactions. Tolperisone has a note: do not use if the activity requires concentration and quick reactions. Why such a difference is not clear, because the active substance of the drugs is the same. Just in case, you should be careful not to drive during treatment.

Противопоказания

Milokalm and Tolperisone are not used in such situations:

  1. myasthenia gravis;

  2. idiosyncrasy;

  3. pregnancy and lactation.

With caution – in people sensitive to lidocaine. Tolperisone hydrochloride is similar in structure to lidocaine and also reduces the motor excitability of neurons in the same way. But this fact also works against the drug: in the presence of an allergic reaction to lidocaine, there may be a similar reaction to tolperisone.

Conclusions

To sum up:

  1. Mydocalm and Tolperisone are centrally acting muscle relaxants. They relieve muscle spasm and thereby break the vicious circle of “pain – spasm – pain.” They are used in combination with NSAIDs for the treatment of diseases of the musculoskeletal system and the central nervous system.

  2. Mydocalm and Tolperisone contain one active ingredient, but the Hungarian Mydocalm is the original remedy, and the Russian Tolperisone is a generic. Clinical trials have shown the high efficiency of the original Mydocalm. Comparative studies of drugs have not been conducted, and so far it cannot be said unequivocally that generic Tolperisone is as good as the original Mydocalm.
