Hello, I am 42 years old, I am a man and I have been struggling with recurring abdominal and back pains for a long time. Doctors cannot tell what is causing the pain and treat it blindly. Sometimes they say it is renal colic, other times it may be caused by pains problems with the pancreas. So I don’t know what’s wrong with me, and I’d like to get rid of this pain completely. So I am asking for help, which means pain in the lower abdomen and back?

I have read a lot about pancreatitis pains and I have the impression that they are similar to mine. Maybe I will write about the pain I feel. Sometimes it grabs so hard that I can’t touch the lower abdomen. The pain seems to radiate to my back, but when it occurs I don’t know which side. Lying down doesn’t help, walking doesn’t help, and sitting doesn’t help. The visit to the hospital ends with basic examinations, administration of antispasmodics in a drip and killing the inflammation. However, sorrows return. Pain in the lower abdomen in men it can have many different causes and can be caused by:

– cystitis – usually accompanied by more frequent urination, burning sensation during voiding or blood in the urine, may also be accompanied by general symptoms such as fatigue, fever. Diagnosis is usually based on symptoms and with a general urine test and culture, treatment is antibiotic therapy;

– orchitis can also be manifested by pain in the lower abdomen radiating from the testicles. If you feel lumps on the testicles, skin changes over the testicle / testes, or urethral leakage, see your GP;

– prostatitis – also manifested by frequent urination of small amounts of urine, urination in droplets. Treatment, depending on the etiology, is symptomatic or with the use of antibiotics;

– appendicitis – the pain applies especially to the right lower abdomen, it is also accompanied by decreased appetite, nausea and vomiting, the occurrence of fever. Pain is strongest when palpating. Diagnosed by imaging studies and laparoscopic surgery is the treatment of choice;

– kidney stones – the pain may occur in the lower abdomen, groin or lower back, most often unilaterally and is colic in nature, it is associated with the passage of deposits through the urinary tract. It can also manifest as haematuria, sometimes requiring hospitalization, for example if you have fever or hydronephrosis. For diagnostics, imaging tests are used to visualize the presence of deposits in the urinary tract, including abdominal CT scan or ultrasound, which allows for the diagnosis of nephrolithiasis.

– acute pancreatitis – in addition to epigastric pain radiating to the back, 90% of people also experience nausea and vomiting. Serum amylase levels are most commonly used to confirm acute pancreatitis, and a result that exceeds three times the upper limit of normal is specific for ACE in 85-98% of cases.

Depending on the laboratory and imaging tests performed, the cause of the pain can be found, while the description of the pain itself can only be used to suspect, and not confirm the diagnosis.
