The doctor explains the causes of the elevated bilirubin
Bilirubin is a bile pigment that results from changes in the heme. Main source of bilirubin is heme that comes from hemoglobin, a pigment found in red blood cells. It circulates in the blood free bilirubin (otherwise unconjugated) bound to protein with albumin.
As a result of changes in the liver, bound (or conjugated) bilirubin is produced. In this form, bilirubin is excreted into the urine and gastrointestinal tract. It means conjugated bilirubin concentration (conjugate) and integer. By subtracting the concentration of conjugated bilirubin from the total, we get the result free bilirubin concentration (unconjugated).
Incorrect concentrations may apply to any fraction. Normal bilirubin levels total bilirubin is in the range of 0,3-1 mg / dl, conjugated bilirubin 0,1-0,4 mg / dl, and free (unconjugated) bilirubin 0,2-0,8 mg / dl. Depending on the bilirubin fraction in which the abnormalities occur, we distinguish different causes of their occurrence.
Conjugated bilirubin increased (conjugated) or mixed hyperbilirubinemia suggests pathology in the liver or biliary tract. The causes may be acute and chronic hepatitis (including those caused by excessive alcohol consumption), cirrhosis, toxic liver damage, organ ischemia, metabolic disorders, primary and secondary neoplasms localized in the liver, and any abnormalities of the biliary tract.
The predominance of free bilirubin may indicate its overproduction as a result of excessive destruction of blood cells (haemolysis), impaired bilirubin uptake, or disturbance of bilirubin conjugation in the liver. Bilirubin conjugation is impaired, among others, in Gilbert’s syndrome. Please consult the test results with your GP. In addition to the test results, it is necessary to read the data from the interview and examine the patient. The doctor will decide on the need for further diagnosis based on the entire clinical picture.
