Hello, I’m 39 years old and I’ve been big lately problems with the body’s immunity. The problem manifests itself through frequent catching colds – runny nose and coughing, I have the impression that they accompany me every day and I cannot get rid of them. It became so tiring that I decided to do something about it and instead of treating myself for another cold, I decided to do a blood count. In the results I have information about immature granulocytes. What do they mean immature granulocytes?

I’ve never experienced anything like this before and I don’t know if it means something serious? Or is it a symptom of some serious illness? Apart from the common colds mentioned above, I cannot complain about other ailments. Of course, I will report the results to the doctor, but I would like to have at least some minimal idea what the immature granulocytes are associated with? My husband says I’m exaggerating, that I shouldn’t worry too much and be hysterical. However, I worry terribly, because it is my health. Please help me and explain what immature granulocytes mean. The doctor explains what the immature granulocytes indicate

Any result of an automatically counted morphology in which they occur immature forms of granulocytes should be repeated as a blood smear test, preferably by manual. Leukocyte cells will then be examined by a laboratory diagnostician who will be able to determine exactly what immature forms are present in the peripheral blood.

The appearance of immature granulocytes as a result of morphology indicates a “shift to the left”, indicates an increased ratio of young neutrophils to multi-segmented, ie mature, neutrophils. The reason for this is the increased production and release of white blood cells from the marrow into the blood before they are fully mature. Most often this happens when an infection or inflammation is present in the body. The appearance of such forms in the peripheral blood may also be a symptom of bone marrow diseases, i.e. leukemia. Based on how big it is the number of immature granulocytes in relation to their mature forms, we may suspect acute or chronic myeloid leukemia. In some cases, a frame shift may also occur following stimulation of bone marrow with growth factors, e.g., G-CSF.

However, not always the presence of immature granulocytes in morphology is the reason for initiating Leukemia diagnosisbecause most of the time it’s just the result of an infection. Among granulocytic cells, blasts are most likely indicative of leukemia, as they indicate bone marrow overload with granulocyte precursors. Depending on the percentage of immature granulocytes in relation to the entire population of white blood cells, a morphology with a smear is performed, and further management is dependent on this. If there is a strong suspicion of a hyperplastic background of laboratory abnormalities, a bone marrow aspiration biopsy is recommended.

The morphology result should be shown to the ordering physician or to the GP, if there is the slightest suspicion of a proliferative disease, the doctor will order the tests necessary to exclude it.
