Our yesterday’s attempts to obtain a referral for a free coronavirus test through the Ministry of Health website have failed. Despite the fact that close contact with the infected and a few serious symptoms were marked on the form, the system decided that the risk of infection was low. We tried again today. We enter the same answers and check if the algorithm will be more kind to us.

Before we get to the questionnaire – some information

By definition, the form that we fill in ourselves was supposed to make life easier for both patients and GPs, who had to refer us to a test so far. Since yesterday, anyone who has difficult contact with their doctor and has contacted an infected person or already has symptoms of COVID-19, can apply for a referral via the gov.pl/dom website.

The website is clear and it is easy to find the link to the questionnaire. Click on “SARS-CoV-2 test qualification form” and we are redirected to the information page.

  1. How does the COVID-19 test qualification form work?

We will learn from it that in order to complete the form, we must have the suspicion of risk of coronavirus infection 18 years of age or older. Let’s also get to know the path that must be taken in order to have a test taken in the event of qualification.

After issuing the order, we will receive an SMS with the proposed place and time of the examination, thanks to which we will avoid queuing at the swab point. However, we will be able to read the test result on our Patient Online Account.

The Ministry also encourages us to submit the form between 8 a.m. and 18 p.m. (also on weekends), as these are the working hours of Home Medical Care consultants who contact us and issue orders. If we fill out the form after 18, we will have to wait until the next day for referral.

COVID-19 Test: The form is six questions

The blue button at the bottom of the page takes us further, and this is where the actual form begins. Two questions await us here. About whether or not we stayed close (face to face) to the infected person for more than 15 minutes. and whether we had direct physical contact with such a person. We mark the answers “yes” or “no”. Next we need to determine if we are suffering from any chronic diseasessuch as, for example, cancer, obesity, diabetes, cardiovascular disease or neither. The next questions are about the symptoms. The ministry is primarily interested in three main ones: cough, fever i dyspnoea. We choose the answers “yes” and “no”. Other typical COVID-19 symptoms such as fatigue, headache, nausea, impaired taste or smell, or none of the above are listed below. Finally, you must provide information whether you are a medical profession.

Today, the editors of Medonet chose the same symptoms that we saw yesterday, i.e. no shortness of breath, but fever, headache and fatigue, and of course contact with the infected. This time, the algorithm decided that “the risk of SARS-CoV-2 infection is high” and qualified us for the test.

To contact a consultant, you must now complete the next form. We provide there your personal and address details as well as a telephone number. The system also asks us if we have a STOP COVID application.

To complete the procedure, we still need to give four consents (the small print informs us that we have to select at least 4 answers). It is about consent to the processing of data contained in the form, telephone contact to the number provided by us in order to generate an order for the test and to declare that we are 18 years of age or older. Interestingly, we must also consent to the transfer of data to the employees of the PZU Zdrowie SA hotline who will issue an order for the test.

Filling out the form takes a few minutes, and once we have sent it, all we need to do is wait for a call from the hotline and remember that the system automatically quarantines us when we receive a test referral.

Note: The form has been designed to go through the one-way step by step procedure, so once we start filling it out, we can’t go back and change or supplement our answers. However, after clicking the input link again, we are directed to the place where we left off.
