What is Trittico CR (Angelini)?

Trittico CR are prolonged release tablets, which are available by prescription in three doses: 150 mg (20 tablets), 150 mg (60 tablets), 75 mg (30 tablets). The main active ingredient is Trittico CR trazodone. Each prolonged-release tablet contains 75 mg of trazodone hydrochloride, equivalent to 68,3 mg of trazodone.

The second variant, on the other hand, are extended-release tablets, which contain 150 mg of trazodone hydrochloride, which is equivalent to 136,6 mg of trazodone. Trittico has a characteristic incision, which makes it easy to divide the lozenge into 3 equal doses.

Trittico CR – destiny

The properties of Trittico CR are based on the action of blocking the reuptake of serotonin by the nerve cell just before the synapse (a junction of two neurons). Trittico CR – dosage

Trittico CR should not be taken in any dose by children and adolescents under 18 years of age, as they may have a negative impact on their psychophysical development. Adults can take Trittico CR orally after a meal. The starting dose of Trittico CR is 75-150 mg / day at bedtime. The maximum dose is up to 300 mg / day given in two divided portions. In a hospital setting, this maximum dose is up to 600 mg / day.

The recommended dose in the elderly, including the very elderly or in debilitated patients, should initially be 100 mg / day in several approaches, or one taken at bedtime. The dose of Trittico CR may be increased after consulting your doctor, depending on how the patient responds. A maximum of a patient can take 300 mg of the drug per day.

Trittico CR – contraindications to use

Trittico CR should not be used in case of hypersensitivity to any component of the preparation. People who are under the influence of alcohol or sleeping pills and patients with acute myocardial infarction should also be careful about this drug. Trittico CR – precautions

Special precautions should be taken when taking Trittico CR in children and adolescents under 18 years of age. This medicine is not indicated for this age group, as clinical trials show a greater frequency of suicidal thoughts and behavior. After consuming this medication, cognitive, behavioral and maturation disorders may occur despite the doctor’s warnings.

Taking Trittico CR may cause orthostatic hypotension or somnolence in the elderly. The effects can be cumulative when combined with other anti-depressants or antihypertensives. When administering Trittico CR to the elderly, various risk factors resulting from comorbidities should also be taken into account. In such situations, the physician should know the patient’s entire medical history in order to exclude the risk.

Trittico CR may be dangerous in the initial phase of use in people suffering from depression and having thoughts of suicide. Depression is in many cases associated with a high risk of self-harm and even suicide. This risk continues until the patient has fully healed. Improvement in the health of a depressed person may not occur in the first few weeks of taking Trittico CR, therefore the patient should be closely monitored. Clinical trials show that the risk of suicide most often occurs at the very beginning of treatment.

Trittico CR should be used with caution in the elderly, as they may experience more orthostatic hypotension, somnolence, and other effects of a cholinolytic agent. The use of this drug by people with epilepsy should also be constantly monitored by a doctor or caregiver. Avoid rapidly tapering or increasing the dose of this drug in people with epilepsy. People with hyperthyroidism and liver and kidney failure must also watch out for Trittico CR, especially in the acute course of these diseases.

Trittico CR must be carefully dosed in people: with heart diseases, eg angina pectoris, after a recent myocardial infarction; with diagnosed circulatory diseases; with disorders in urination, e.g. prostatic hyperplasia, acute narrow-angle glaucoma or increased intraocular pressure. The use of antidepressants in patients with schizophrenia and other psychotic disorders may lead to an increase in psychomotor symptoms and paranoid thoughts. When treating men, priapism may occur – a painful and prolonged erection without sexual stimulation.

Trittico CR – interactions with other drugs

The main component of Trittico CR – trazodone, may increase the sedative effect of drugs: hypnotics, antihistamines, anxiolytics and antipsychotics. The dosage should be adjusted to the individual needs of the patient, and even discontinuation of the drug in the event of worsening symptoms. The processing of antidepressants such as Trittico CR by the liver may be accelerated if the patient is taking oral contraceptives, phenytoin, carbamazepine, or barbiturates. It is also worth remembering that some antipsychotics and cimetidine can inhibit the metabolism of antidepressants.

The parallel use of Trittico CR with strong inhibitors of CYP3A4 must be avoided. Why? Because an interaction may occur between the antidepressant and the group of inhibitors such as erythromycin, ritonavir, ketoconazole, indinavir, itraconazole and nefazodone. Clinical trials show that CYP3A4 inhibitors may increase the plasma concentration of trazodone. Combining Trittico CR with these drugs may cause hypotension, fainting and nausea.

Taking Trittico CR together with tricyclic antidepressants is also not the best combination. Concomitant use of these preparations may increase the risk of drug interactions. Then there is a high probability of side effects on the cardiovascular system, and a serotonin syndrome may also occur.

The group of scientists also observed that combining trazodone with monoamine oxidase inhibitors (MAOIs) may result in unwanted interactions between these substances. It is not recommended to use trazodone during treatment with MAO inhibitors or within about two weeks after the end of therapy. Conversely, the use of MAO inhibitors for a week after discontinuing trazodone is also not recommended.

During drug therapy with carbamazepine drugs, they cannot be combined with antidepressants. The simultaneous use of Trittico CR and carbamazepine reduces the concentration of trazodone and its active metabolite m-chlorophenylpiperazine in the blood plasma by 76% and 60%, respectively. During treatment, you need to be more vigilant when combining Trittico with anesthetics and skeletal muscle relaxants. This combination increases their effect and can be dangerous.

Trittico CR – side effects

Trittico, like other medicines, can cause various side effects. First of all, it can cause: abnormal blood test results, allergic reactions and anorexia. The use of this preparation may cause weight loss. Trittico CR can also cause insomnia, hallucinations, nightmares and aggressive behavior.

Rash, itching and excessive sweating may occur when using this remedy. Side effects of using Trittico CR may also include various types of swelling, general body weakness, fatigue, stinging in the chest and flu-like symptoms.

Effect of Trittico CR on pregnant women and breastfeeding

Pregnant women, especially in the first trimester, should avoid Trittico CR, because the drug in large amounts may lead to miscarriage. Reasonable dosage of this drug in consultation with a doctor in the later stages of pregnancy does not affect either the delivery or the development of the baby. If a woman is taking trazodone until delivery, it is worthwhile to monitor the newborn baby after birth for any symptoms of ‘withdrawal’.

Clinical studies show that the trazodone component of Trittico CR may pass into breast milk in small amounts. The amount of long-acting substances is small, but it should not be underestimated. Trittico CR – drug overdose

The most common symptoms of an overdose with Trittico CR are dizziness, sleepiness and vomiting. In more severe cases, coma, convulsions, tachycardia, hypotonia and respiratory depression may occur. All of these symptoms may appear up to 24 hours after the overdose, sometimes even later. Too high a dose of trazodone in combination with antidepressants can lead to serotonin syndrome.

What should you do if you have overdosed on Trittico CR? The dose of activated charcoal should be administered within one hour of the overdose. Gastric lavage or induction of vomiting may be considered in adults. A patient who has overdosed on an antidepressant should be observed for at least 6 hours after the event occurred. Then you need to control the heart rate, blood pressure and the level of awareness of the victim.
