Researchers assessed the clinical and laboratory characteristics of 11 patients from Germany and Austria who developed thrombosis or thrombocytopenia after vaccination with AstraZeneka. The study revealed the mechanism of their formation.

Thrombosis and thrombocytopenia following AstraZeneki vaccine. Findings

Of the 11 patients, 9 were female, with a median age of 36 years (range 22 to 49). Beginning 5 to 16 days post vaccination, patients experienced one or more thrombotic events, with the exception of 1 patient who experienced fatal intracranial haemorrhage. Among these patients, cerebral vein thrombosis and pulmonary embolism were diagnosed, among others. 6 patients died. Five patients developed disseminated intravascular coagulation.

Vaccination with AstraZeneką may cause the rare development of thrombotic thrombocytopenia mediated by antibodies that activate platelets. The mechanisms of events are very similar to heparin induced thrombocytopenia.

Due to the lack of heparin intake in the history of patients, the new complication was called VITT (vaccine-induced immune thrombotic thrombocytopenia), i.e. immune thrombosis with vaccine-induced thrombocytopenia.

Scientists have detected antibodies that trigger the clotting reaction

These reports were confirmed by Norwegian research. Both teams found that the patients had antibodies that trigger a clotting reaction, thereby using up platelets, leading to potentially fatal strokes or embolisms.

The findings have several important clinical implications. First, clinicians should be aware that in some patients venous or arterial thrombosis may develop in atypical sites such as the brain or abdomen, which becomes clinically apparent about 5 to 20 days after vaccination. If this reaction is accompanied by thrombocytopenia, it may be an adverse outcome of vaccination. Fortunately, cases are quite rare (about 1 in 100).
