Autumn is a time when special attention is paid to the subject of gynecological cancers. October is Breast Cancer Awareness Month, September was Gynecological Cancer Awareness Month. Due to biological conditions, women are much more exposed to cancer than men. This is why it is worth talking about the related risks and prevention throughout the year.

Breast cancer – the most common gynecological neoplasm
Neoplasms that can be diagnosed by a gynecologist, i.e. breast, ovary, uterus, vagina, cervix and vulva, account for as much as 50 percent. all cancers that occur in women. Breast cancer is by far the most common gynecological cancer. Statistically, non-smoking women most often die because of it.
“Together, it accounts for a quarter of all cancers in women,” says the drug. Andrzej Zmaczyński. – This disease affects patients most often after 50 years of age. The prophylactic program in this case is mammography, which is performed in Poland every two years between the ages of 50 and 60.

At the same time, it does not mean that also younger women in their twenties cannot get sick with it.
– In their case, preventive examinations are not performed. The only tests indicated for them are breast self-examination and ultrasound performed every two years (refunded by the National Health Fund after referral from a gynecologist). In addition to the aforementioned tests, sometimes a mammotomy biopsy is also performed, during which any changes can not only be diagnosed, but also removed immediately. Although it is the most common gynecological neoplasm, it is not the easiest to detect – explains the gynecologist.

As for the risk factors of this disease, the use of hormonal contraception can be mentioned among them, although it increases the probability of the disease slightly, i.e. by about 1-2 percent. Interestingly, recent studies prove that taking hormonal pills reduces the incidence of endometrial and ovarian cancer. On the other hand, breastfeeding has a protective effect against the occurrence of this cancer.

When it comes to treatment, it does not always have to end with a mastectomy, which is something many women fear. What’s more, nowadays it even happens rarely. Unfortunately, in Poland the disease is still very often diagnosed only in the advanced stage (in this respect Poland is very poor in comparison with Europe), and then it is necessary to have the breast excised. The most common, however, are conserving surgeries, locally excising the tumor or performing a quadrantectomy.
– The mortality associated with breast cancer would be much lower if the disease had been detected earlier – says Marcin Mika, MD, PhD, gynecologist. – It is treated relatively well, but only if it is diagnosed early. To the extent that a full recovery is possible even in 95%. early detected cases.
Cervical cancer – the most easily detectable gynecological neoplasm
It can be said that cervical cancer is one of the most easily detectable gynecological neoplasms. As in the case of breast cancer, mammography after 50 years of age is a validated screening method, here it is cytology and HPV testing.
– In the case of cervical cancer, it is possible to detect even pre-neoplastic lesions, which is often used among patients from the highest risk group – says Marcin Mika, MD, PhD. Therefore, a properly conducted screening program and vaccination against the HPV virus would allow the vaccinated population to get rid of this cancer almost completely. In Poland, however, this is not done, because the national vaccination program, reimbursed by the National Health Fund, has unfortunately been abandoned.
Although the HPV test is not reimbursed, the cytology itself is. Every woman should know about it and have regular check-ups.
Ovarian cancer – the most insidious gynecological cancer
Ovarian cancer is referred to by gynecologists as the most insidious gynecological cancer. In the early stages of development, it practically shows no symptoms, in fact it only becomes noticeable at an advanced stage. Symptoms, if they begin to appear, are usually non-specific, associated more with gastric ailments (flatulence, feeling of fullness or diarrhea) than gynecological ones.
This is why it is usually detected too late or even by accident during other treatments, which translates into poor treatment results. Also, the exact risk factors are not known for this type of cancer.
– There is also, unlike breast or cervical cancer, virtually no screening tests that would enable it to be found in a woman’s body – says specialist Marcin Mika, MD, PhD. He also adds that it is the insidious nature of the disease that causes the very high mortality due to it.
In the case of ovarian cancer, the basic diagnosis is a two-handed gynecological examination through the vagina and rectum, and imaging (transabdominal and transvaginal ultrasound). Complementary tests are also computed tomography and magnetic resonance imaging.
The diversity of gynecological cancers is very large and the list of them is really long. Every woman should be aware of the potential risk, regularly taking advantage of preventive examinations, as well as those not reimbursed by the National Health Fund. In a large proportion of cancer cases, it is possible to fully cure them, but the condition is early detection.
