The stair test will evaluate how your heart is working
Research by Spanish cardiologists has shown that climbing the stairs to the fourth floor in less than a minute indicates that the heart is in good condition. The results were presented at Best of Imaging 2020, the scientific congress of the European Society of Cardiology.
‘The stair test is an easy way to check the health of your heart,’ said study author Dr Jesús Peteiro, a cardiologist at Coruña University Hospital in Spain. – If it takes you more than a minute and a half to climb four floors, your health is not optimal and it is worth consulting a doctor.

Researchers wanted to investigate the relationship between daily activities – such as climbing stairs – and the results obtained from exercise testing. ‘It was about finding a simple and inexpensive method to assess the health of your heart,’ explained Dr Peteiro. – It can help doctors in selecting patients for more detailed examinations.

What was the research like?
The study involved 165 patients referred to an exercise test due to diagnosed or suspected coronary artery disease. The most common symptoms were chest pain or shortness of breath with exertion. Participants walked or ran on the treadmill, gradually increasing the intensity of the exercise until exhaustion.
The participants’ physical capacity was measured and then – after a rest of 15 to 20 minutes – they were asked to enter four floors. The patients had a total of 60 steps to climb, the pace was supposed to be quite fast, without stopping, but also without running up.
- Infectious heart disease
The researchers then analyzed the relationship between MET (metabolic equivalent) obtained during exercise testing and the time needed to climb the four floors.
MET, or metabolic equivalent, is used to describe the intensity of exercise. One MET is the energy expenditure while resting, more specifically the oxygen consumption during a resting in a sitting position for a person weighing 70 kg.
Patients who climbed the stairs in less than 40-45 seconds had more than 9-10 METs in the stress test. Previous studies have found that 10 METs during exercise testing are associated with lower heart-related mortality (1% or less per year, or 10% over 10 years).
Conversely, patients who took 90 seconds or more to climb stairs previously had less than 8 METs. This translates into a mortality rate of 2-4%. annually or 30 percent. in 10 years.
- The seven worst heart conditions in adults
During the treadmill exercises, the researchers also assessed the work of the heart. If it was working normally, it indicated a low probability of coronary artery disease. They later compared these findings with the results of climbing the stairs. About 58 percent. of patients who climbed the stairs in more than 90 seconds showed heart abnormalities while exercising on the treadmilli. In the group of people who climbed the 4th floor in less than a minute, abnormalities during exercise on the treadmill occurred in 32%.
