On Friday, Health Minister Adam Niedzielski announced that the regulation on this subject would be published in the evening of that day.

– On Sunday, we want to provide a new form, because this form for children, i.e. for people aged 16-18 must have an additional element in the form of parental consent and there is a separate section devoted to this consent, where you must enter basic data and provide information about consent – explained the minister.

He recalled that the registration for teenagers is scheduled to start on Monday. – I recommend that all interested persons go to the website of the Ministry of Health and download this form in order to prepare for vaccination, and of course register on Monday – he added.

Adolescent vaccinations – complete the questionnaire

– On Monday, registration for vaccinations against COVID-19 for people aged 16-17 starts – says Deputy Minister of Health Anna Goławska.

– You must complete the “Pre-screening interview questionnaire before vaccinating a minor against COVID-19” prior to visiting a vaccination center. It will be available for download from Sunday at www.gov.pl/szczepimysie. We encourage you to download the form in advance and sign it by your legal guardian. Of course, if someone does not have a printed questionnaire with them, they will receive it to be completed at the Vaccination Center. Then the minor will have to come to the vaccination with a legal guardian who agrees to provide benefits – explains Gocławska.

«The questionnaire for the initial screening interview before vaccinating a minor against COVID-19 is completed in the case of vaccination of a person who is under 18 years of age on the day of vaccination. It is filled in by the vaccinated person and their legal guardian.

The version of the questionnaire for minors includes an addition to the consent of the legal guardian to vaccinate the minor. In the case of vaccination of a person over 16 years of age the questionnaire also requires a signature expressing consent to vaccination against COVID-19 ″ – we also read on the government’s vaccination website.
