The Polish Society of Epidemiologists and Doctors of Infectious Diseases, a national consultant in the field of infectious diseases and the Chief Sanitary Inspector issued recommendations on the management of patients infected with SARS-CoV-2 who do not require hospitalization and can be treated in home isolation.
How to deal with COVID-19 patients who do not require hospitalization?
The number of people infected with the SARS-CoV-2 coronavirus in Poland has already exceeded 1000 people. Some of them pass the disease mildly, others require hospitalization. The Chief Sanitary Inspector, in consultation with experts in the field of infectious diseases, prepared recommendations on how to deal with people who can be treated in conditions of home isolation.
The GIS recommends that a patient’s location with COVID-19 be arranged with him prior to discharge. The patient is discharged with the code B34.2 or U07.1 and receives a sick leave for at least 14 days. If the patient can be isolated at home, the patient should be delivered to the place of residence by a dedicated means of transport. However, the patient may not always be in isolation in his own home. Then it should be transported to a place of outdoor isolation designated by the voivode.
B34.02 is a code that means: Coronavirus Infections, Unspecified
U07.1 is the code that stands for COVID-19
Both codes are included in the International Statistical Classification of Diseases and Health Problems – ICD-10.
Further recommendations relate to the re-sampling of a patient infected with the SARS-CoV-2 coronavirus. «After 10-12 (for children 14) days from the onset of symptoms, and for asymptomatic patients 10-12 days (for children 14) from the first time the smear showing infection is taken, the sample taker collects a control swab in the place of isolation indicated by Sanepid ».
Check: Home Quarantine Policy
If the test is negative, the control test is performed after a minimum of 24 hours. If the result is negative twice, the patient can be released from isolation, with an order to maintain special hand hygiene for a period of at least seven days, due to the longer persistence of the coronavirus in the stool.
In the case of children, after obtaining a double negative result, they should not participate in nurseries, kindergartens and schools for the next week. Currently, all educational institutions are closed, and children still learn at home.
In the event that any of the control test results are positive, the tests should be repeated seven days apart until a negative result is obtained.
Recommendations for patients staying in home isolation as a result of infection with SARS-CoV-2
According to GIS, the goal of home isolation is the isolation of a patient with a positive COVID-19 test result, which, after being examined by a doctor, does not require hospitalization due to the state of health.
Home isolation is used in patients who are tested positive for SARS-CoV-2 coronavirus and asymptomatic or who show mild to moderate symptoms of COVID-19 (e.g. low-grade fever, cough, sore throat, weakness).
GIS also reminds that people from home contact with sick people have the highest risk of infection because it occurs through droplets or when touching contaminated objects.
If the condition of a person staying in home isolation worsens, an ambulance should be immediately called and informed about the diagnosis of COVID-19 and home isolation. Deterioration of health may include high and persistent fever, shortness of breath, chest pain, epistaxis, and skin ecchymosis.
During home isolation, a patient with COVID-19 should stay in one room XNUMX/XNUMX, where no other household members or even pets may stay. Household members should not approach a person in home isolation within a distance of two meters. Pets, although they cannot and do not infect themselves, can transmit the coronavirus to the hair when they come into contact with someone who is sick.

A person in home isolation should have their own cutlery, and any items they use should be thoroughly washed and disinfected. The room where the sick person is staying should be ventilated frequently. Fans, air conditioning and air purifiers should not be used.
What is important, all household members are quarantined throughout the isolation period and for 14 days after the infected person is considered healthy. During this time, people in quarantine are not allowed to leave the house, do shopping, etc. They can move only in rooms where the sick person is not isolated, they can also go out to their own home garden, but without contact with the sick person or their neighbors.
All guidelines are available on the website of the Chief Sanitary Inspectorate.
