Pyralgina – indications
The indication for the use of Pyralgina is:
- severe pain;
- fever – especially when other measures are ineffective.
When we feel an elevated body temperature and we start to feel other symptoms of a cold, it is worth taking the temperature and watching how it changes. The Geratherm Classic mercury-free thermometer can be used for this purpose. It is a safe solution for health and the environment.
Pyralgina – contraindications
Pyralgine should not be used in every situation and by every person. The contraindication to the use of Pyralgina is:
- hypersensitivity to metamizole, other pyrazolone and pyrazolidine derivatives (e.g. propyphenazone, phenazone, or phenylbutazone) or any other ingredient of the drug;
- analgesic asthma syndrome;
- diagnosed intolerance to painkillers, manifested by urticaria, angioedema, bronchospasm or other anaphylactoid reaction to paracetamol, salicylates and other non-opioid analgesics (including NSAIDs);
- acute renal or hepatic failure;
- acute hepatic porphyria;
- congenital G-6-PD deficiency;
- bone marrow dysfunction and changes in blood count, e.g. leukopenia, agranulocytosis, anemia;
- third trimester of pregnancy;
- taking drugs whose active substance is a pyrazolone or pyrazolidine derivative.

Pyralgina – dosage
Pyralgina is approved for use by adults and adolescents over 15 years of age who weigh over 53 kg. Pyralgine should be taken orally with or immediately after a meal. The tablet should be washed down with water.
Pyralgina dose depends on the severity of the pain or fever. A single maximum dose of up to 1000 mg of metamizole, i.e. 2 Pyralgina tablets. However, the maximum daily dose is 4000 mg, i.e. 8 tablets. This means that you can take a maximum of 1-2 tablets up to 4 times a day at one time. Leave a maximum of 6-8 hours between each dose. It is also worth remembering to always use the smallest possible amount of medication to lower your temperature or relieve pain.
Pyralgina starts working after 30-60 minutes after administration. However, the drug can be used for a maximum of 3-5 days. If there is no improvement after this time, contact your doctor. Pyralgine can not be used for more than 7 days.

Pyralgina – precautions
Not every person can use Pyralgina. Before using Pyralgina, discuss the following issues with your doctor, precluding the use of the drug:
- low blood pressure, diseases of the heart, circulatory system, constriction of intracerebral vessels, low circulating blood volume, dehydration;
- high fever;
- renal or hepatic failure;
- gastric ulcer and duodenal ulcer;
- bronchial asthma (polyps of the nasal mucosa and sinuses);
- allergic diseases (atopy, urticaria);
- intolerance to certain dyes and preservatives;
- alcohol intolerance.
Especially the elderly, with a reduced value of creatinine clearance, or weakened people should be careful. They should use a lower dose of Pyralgina due to the longer excretion of drug metabolism products from the body. Kidney or liver disorders slow this process down. The limitations should be remembered especially when using Pyralgina for a long time. However, remember not to use high doses many times. Unfortunately, the effects of long-term use of Pyralgina in this group of people are not known.
Pyralgine should also not be used in children under 15 years of age due to the high dose of the active substance.
In addition, it is worth remembering that 1 tablet of Pyralgina contains 34,5 mg of sodium, which corresponds to 1,7%. the daily dose for an adult. Some ingredients of the drug can lead to an allergic reaction, so stop taking the drug immediately and call for help if you notice the following symptoms:
- shortness of breath;
- swelling of the face, tongue and throat making it difficult to breathe and swallow;
- itching, hives, reddening of the skin or mucosa;
- weakness, fainting or pressure drops – symptoms of anaphylactic shock.
Caution should be exercised by people who are allergic to other pyrazolones and pyrazolidines, because reaction after applying Pyralgina it may be similar. People with asthma and atopy should also be careful, as other allergies increase the risk of anaphylactic shock.
You should contact your doctor immediately if you develop:
- symptoms of agranulocytosis – fever, chills, swallowing problems, ulcers in the mouth, nose, sore throat;
- pancytopenia – symptoms of blood dyscrasia;
- Stevens-Johnson syndrome;
- necrotic separation of the epidermis.
Pyralgina may induce hypotensive reactions, i.e. lower blood pressure. The risk largely depends on the amount of the dose taken. The main influence is:
- heart failure and circulatory disorders;
- systolic blood pressure less than 100 mm Hg;
- low circulating blood volume;
- high fever;
- dehydration.
People with the above diseases should use Pyralgina under strict medical supervision and only if it is absolutely necessary.
Pyralgina has no influence on driving vehicles and the use of machines. However, if the patient uses higher doses than recommended by the manufacturer, these abilities may be impaired.

- liver cell damage;
- nephritis;
- allergic bronchospasm;
- rash, hives, swelling of the skin;
- exfoliative inflammation and toxic epidermal necrolysis;
- abnormal blood test results, e.g. decrease in white blood cells due to bone marrow damage (leukopenia, agranulocytosis), decrease in blood platelets such as thrombocytopenia, aplastic anemia;
- red color of urine as a result of taking too high doses of Pyralgina.
Pyralgina – side effects
Side effects from the use of Pyralgina are reported quite rarely and it should be remembered that they will not occur in every person. However, you should be vigilant to seek medical help in time. Among the quite rare side effects of using Pyralgina, the following should be mentioned:

- large drops in blood pressure without symptoms of hypersensitivity – at the same time there is a high fever.
Rare side effects include:
- leukopenia;
- anaphylactoid and anaphylactic reactions;
- rashes;
- delicate skin and mucosal lesions such as itching, swelling, hives, burning etc .;
- shortness of breath;
- gastrointestinal disorders.
It should be remembered that these side effects may appear with varying severity, as well as take severe forms with generalized urticaria, bronchospasm, angioedema, pressure drop, cardiac disorders, and even severe anaphylactic shock.
To very rare side effects of using Pyralgina belong:
- agranulocytoza;
- thrombocytopenia;
- sudden renal dysfunction (proteinuria, oliguria, anuria appear);
- interstitial nephritis;
- toxic epidermal necrolysis;
- asthma;
- Stevens-Johnson syndrome;
However, the frequency of the following side effects of using Pyralgine is unknown:
- aplastic and hemolytic anemia;

Pyralgina — alcohol
Pyralgina interacts with alcohol. After drinking alcohol, the concentration of the active substance and its metabolites may change. By drinking alcohol, patients taking NSAIDs increase the risk of gastrointestinal bleeding and kidney problems.
Cases of acute renal failure have also been reported in patients abusing high-alcoholic drinks and taking NSAIDs. Therefore, the manufacturer recommends that you refrain from drinking alcohol while using Pyralgina.
Pyralgina – pregnancy
Pyralgina is not recommended for pregnant women throughout their pregnancy. However, there are exceptional situations when the doctor allows the administration of single doses of the drug. This is because there is no other treatment available, but they only apply to the first and second trimesters. In the third trimester, Pyralgina is completely banned.
The use of Pyralgina during this period may have a negative effect on health. The drug interferes with kidney function and leads to narrowing of the fetal arterial duct. If a woman accidentally takes Pyralgina in the third trimester, the amniotic fluid and ductus arteriosus should be examined using ultrasound and echocardiography.

Pyralgina – breastfeeding
Metamizole metabolism products go into breast milk in quite large amounts. Therefore, Pyralgina should not be used during breastfeeding as these substances may be harmful to the breastfed baby. If a woman needs to take this medication, she is advised to collect and drain the food for another 48 hours after taking metamizole.
Pyralgina – interactions with other drugs
Pyralgina interacts with other medications. When taking the drug, remember that Pyralgina:
- should not be used with other pyrazolone and pyrazolidine derivatives;
- enhances the effect of oral antidiabetic and anticoagulant drugs, which are a derivative of coumarin, antiepileptic drugs (phenytoin) and antibacterial sulfonamides;
- may have an influence on the increase of the haemotoxicity of methotrexate (a drug used in the treatment of neoplastic diseases) – this applies especially to the elderly, therefore these drugs should not be combined;
- can lead to a decrease in the concentration of cyclosporine in the blood serum – sometimes it is enough to adjust the dose;
- may reduce the effect of acetylsalicylic acid on thrombocyte aggregation – while using drugs, caution should be exercised when acetylsalicylic acid is used in cardiological treatment.
On the other hand, the action of Pyralgina is influenced by barbiturates, which weaken its action, and MAO inhibitors, which enhance its action. In addition, Pyralgina should not be used together with chlorpromazine, as such a combination may lead to hypothermia.
It should also be remembered that metamizole, the active substance of Pyralgina, belongs to the pyrazolones that may interact with captopril, lithium and triamterene. In addition, these substances may interfere with the effectiveness of antihypertensive agents and diuretics. It has not been confirmed whether metamizole, as a pyrazolone derivative, also has this effect.
Pyralgina – storage
Pyralgine should be stored out of the sight and reach of children, preferably in the original packaging. The drug should be stored at a temperature below 25 degrees C.
Pyralgina should not be used after the expiry date which is stated on the box and blister. The expiry date should be understood as the last day of the month specified on the package. After the expiry date, the drug should not be thrown into the sewage system or trash. Expired drugs should be taken to the nearest pharmacy and disposed of in a special container to protect the environment.
