Prestarium is a drug used in cardiology and family medicine to treat hypertension and heart failure. The preparation affects the cardiovascular system – it reduces the load on the heart and lowers blood pressure. Prestarium is available on prescription.

Indications for use of the drug Prestarium
- arterial hypertension (alone or in combination),
- stable coronary artery disease – to reduce the risk of cardiovascular events in patients with a history of myocardial infarction and / or revascularization,
- symptomatic heart failure (in combination therapy).
- Hypertension (monotherapy and combination therapy): initially 5 mg once daily in the morning.
- In patients with a strongly activated renin-angiotensin-aldosterone system (especially with renal hypertension, fluid and electrolyte disturbances, cardiovascular disorders or severe hypertension), the recommended starting dose is 2,5 mg daily. Close medical supervision is required at the beginning of treatment in this group of patients. After about 4 weeks of treatment, your doctor may increase the dose to 10 mg once a day.
- In elderly patients, the starting dose is 2,5 mg daily; After one month of treatment, the doctor may increase the dose to 5 mg, and if necessary, to 10 mg per day. The dose must be adjusted depending on the kidney function.
- Symptomatic heart failure: it is recommended to initiate the use of Coversyl in combination with non-potassium-sparing diuretics and, if necessary, with digitalis preparations and / or beta-blockers, under the conditions of specialist medical care with an initial dose of 2,5 mg 1 once a day in the morning. Your doctor will adjust the dose based on your individual response to treatment. After 2 weeks of treatment, it may increase the dose to 5 mg per day.
- In patients with impaired renal function (creatinine clearance less than 60 ml / min but greater than 15 ml / min), individual dosage adjustment is necessary; in individual cases, the appropriate dose is determined by the doctor. In these people, treatment can only be carried out under strict medical supervision. If the creatinine clearance is less than 15 ml / min and the patient is undergoing dialysis, the dose (2,5 mg) is administered on the day of dialysis after dialysis.
- No dose adjustment is required in patients with hepatic impairment.
- Stable coronary artery disease: 5 mg daily initially for 2 weeks, then 10 mg daily, provided that 5 mg daily is well tolerated. In elderly patients, initially 2,5 mg daily for a week, then 5 mg for another week, and then 10 mg daily. The doctor will only increase the dose when the lower dose is well tolerated by the patient. It is necessary to adjust the dosage depending on the kidney function.

Prestarium, contraindications and warnings
- In the case of severe heart failure, as well as in patients with renal failure, water and electrolyte disturbances, using diuretics or vasodilators, treatment should be started under strict medical supervision.
- In patients who are particularly at risk of developing hypotension, who are dehydrated, have decreased blood volume or have electrolyte disturbances (regardless of whether they have low sodium levels in the blood), your doctor will advise you to correct the existing problems before starting treatment. Blood pressure, blood potassium and kidney function should be carefully monitored.
- There are insufficient data on the safety and effectiveness of the preparation in children and adolescents. For this reason, the preparation should not be used in this age group.
