Preductal MR is a drug used in patients suffering from stable coronary artery disease (also known as angina or angina). Common symptoms associated with the ailment are sudden shortness of breath and a feeling of pain in the chest. This is due to the insufficient supply of oxygen to the heart muscle in relation to the actual demand. The purpose of the trimetazidine in Preductal MR is to reduce the heart’s use of oxygen. What is the exact mechanism of action of the drug and what potential side effects can Preductal MR have?

Active substance: trimetazidine

Preductal MR – action

Preductal MR, in which trimetazidine was used as the active substance, is a drug with the so-called cytoprotective effect, which means that it is designed to protect both the structure and function of cells in various organs. Its protective effect is based on reducing the consequences of insufficient blood supply to the tissues, and thus – reduced oxygen distribution. Trimetazidine lowers oxygen consumption in the body by inhibiting beta-oxidation of fatty acids. As a result of such action, the process of glucose oxidation is intensified. The actual oxygen consumption is reduced, as as many as 26 oxygen molecules are required for the proper course of the beta-oxidation process, and only 5 molecules for the oxidation of glucose. This enables the delivery of much larger amounts of the life-giving element to the heart muscle cells and reduces the symptoms associated with coronary pain.

Preductal MR – indications

Trimetazidine contained in Preductal MR is a drug that is indicated for patients suffering from stable coronary disease. However, this agent is not a drug of first choice and is therefore only used when other drugs are not working or are not adequately tolerated. In addition to the main indication, trimetazidine is also used in the treatment of disorders of the proper functioning of the labyrinth.

Preductal MR – dosage

The usual dosage of Preductal MR is based on taking one tablet of 35 mg of trimetazidine twice daily (every 12 hours). The drug should be taken with meals. However, there are dosage modifications for renal dysfunction (1 tablet containing 35 mg of trimetazidine taken with breakfast). Dosage reduction is also recommended when Preductal MR is used by elderly patients.

Preductal MR – contraindications to use

Preductal MR should not be taken by patients who are hypersensitive to trimetazidine or any of the excipients contained in the tablet (magnesium stearate, povidone, colloidal silica, hypromellose 4000, macrogol, glycerol, E172, E171, calcium hydrogen phosphate). Trimetazidine is also contraindicated in people suffering from movement disorders, e.g. tremors, Parkinson’s disease or restless legs disease – the drug may aggravate these symptoms. Preductal MR should not be used in patients with severe kidney dysfunction. Although no harmful effects on the development of the fetus have been demonstrated, the use of trimetazidine during pregnancy is not indicated. There are no data on the penetration of the drug into breast milk, so it is safer not to use Preductal MR during lactation.

Preductal MR – warnings

Preductal MR is a drug used to treat stable coronary artery disease, and its mechanism of action does not stop angina attacks or myocardial infarction. Movement disorders, such as restless legs syndrome, difficulty walking or parkinsonian symptoms may develop during treatment with trimetazidine – in such a situation, the drug should be discontinued and immediately consulted by a specialist doctor. These symptoms are rare and disappear about 3-4 months after stopping Preductal MR. Caution should be exercised when driving vehicles due to the possibility of dizziness and increased drowsiness after ingestion of the drug.

Preductal MR – interactions

The drug does not show significant interactions with other drugs. However, Preductal MR should be used with caution in combination with medications that counteract high blood pressure. This is associated with an increased risk of syncope, which is usually due to hypotension.

Preductal MR – side effects

The most common side effects of Preductal MR are: headache and dizziness, abdominal pain, dyspeptic symptoms (indigestion), diarrhea, nausea, vomiting, hives, rash, and asthenia (physical and mental weakness).
