Paraphimosis is a complication of phimosis that requires prompt medical attention. The condition is that the foreskin is stuck behind the glans of the penis, in the area of the gastric groove. When the foreskin is trapped in this position, putting pressure on the penis, and it is impossible to pull it back over the glans, we are dealing with an parapostrum. What to do in this situation? When can paraphimosis appear and how is it treated?
What is the parapostrum?
Paraphimosis is one of the complications of phimosis, i.e. narrowing of the mouth of the foreskin, which can occur in children and adults. Phimosis prevents the foreskin from slipping from the glans into the gastric groove, and although in the youngest it is a transient and physiological condition (congenital phimosis), it can also become pathological (acquired phimosis).
So what is a complication of phimosis in the form of a paraphimosis? If there is a problem with sliding the foreskin off the glans, but the foreskin is still forcibly removed, there is a risk that it will not be able to reattach it on the glans. The foreskin wedged in the area of the gut groove and pressing on the penis, accompanied by swelling and pain, is the parapostrum.
What are the symptoms of parapitis?
Paraphimosis is a condition that causes severe pain in the penis and swelling. Glans ischemia, dysuria (painful urination) or haematuria (also known as haematuria) may develop. The parapostrum requires prompt medical intervention, because in extreme cases it may even lead to necrosis at the pressure site.

Shalp – causes
Another phenomenon that affects the male genitalia, phimosis, can lead to paraphimosis. This term is used to describe the narrowing of the mouth of the foreskin, i.e. the skin fold overlapping the glans penis. There are two forms of phimosis:
- congenital phimosis – it occurs in most newborns, usually during the first few years of life the foreskin is long and adheres to the glans (it is stuck to it), completely covering it; it is a physiological state which gradually subsides: to approx. 3-4. the year of life, the mouth of the foreskin begins to widen, and the complete removal of phimosis from the glans should be possible until the age of 17;
- acquired phimosis – it is a pathological condition that concerns the abnormal resolution of physiological phimosis in children, but also occurs in adults as a result of injuries, inflammations, urological procedures, certain diseases or problems with the frenulum.
If, in a few years old child (3-4 years old), the foreskin still does not slide off, do not force it away, but consult a doctor. Also in adolescents or adults, such a situation should be consulted with a specialist, because self-attempting to remove the foreskin may lead to the formation of a parapet.
Direct causes of paraphimosis in men with phimosis it is masturbation, intercourse, or trying to force the foreskin back.
Paraphimosis – treatment
If the foreskin is blocked behind the glans and the flare occurs, you should see a doctor quickly – the problem is painful and the lack of intervention may lead to further complications. In the case of the parapostrum, it is essential in the first place freeing the foreskin from the gastric groovewhich can be done manually or by surgery. Supportive measures for the manual method are hyaluronidase or mannitol (20%). If such a procedure is not effective, the foreskin is incised.
When the paraphimosis problem is resolved, it is still necessary to start phimosis treatment – sometimes conservative treatment is possible, in other cases circumcision is recommended, i.e. shortening the foreskin. Depending on the situation, excision of the foreskin skin can be performed by planning a surgical procedure for a delayed date or by carrying out the procedure simultaneously with the removal of the parapostrum.
What is circumcision?
Circumcision is the surgical removal of the skin of the foreskin under general anesthesia in children, and local anesthesia in adults. The procedure requires proper preparation, consideration of possible contraindications to its performance, and compliance with postoperative medical recommendations for about 4-6 weeks.
