Oranex – indications, contraindications, side effects

Oranex is an antibacterial drug for systemic use. Macrolide antibiotic used to treat bacterial infections. The preparation is recommended in acute bronchitis, pneumonia, and infections of the skin and soft tissues. Oranex is in the form of film-coated tablets or granules for oral suspension. It is issued by prescription.

Oranex – indications for admission

Oranex is a macrolide antibiotic that is used to treat the following infections caused by microorganisms sensitive to azithromycin:

  1. upper respiratory tract infections: bacterial pharyngitis, tonsillitis, sinusitis, acute otitis media,
  2. lower respiratory tract infections: acute bronchitis, exacerbation of chronic bronchitis, mild to moderately severe pneumonia, including interstitial pneumonia,
  3. skin and soft tissue infections: erysipelas, impetigo, secondarily infected pyoderma; migratory erythema (the first symptom of Lyme disease).
  4. Additionally, Oranex in the form of 500 mg tablets is indicated for the treatment of sexually transmitted diseases (uncomplicated infections caused by chlamydia).

Oranex and contraindications

Contraindication to the use of Oranex is allergy to any component of the preparation or to other macrolide antibiotics.

Dosage

Dosage of Oranex 500 mg film-coated tablets is provided below. This form of preparation is recommended for adults and children weighing more than 45 kg.

Adults and children weighing more than 45 kg:

– respiratory tract infections, skin and soft tissue infections (except erythema migrans): 500 mg once daily for 1 days,

– migratory erythema: a single dose of 1 g on the first day, then from the 2nd – 5th day. 500 mg once a day,

– sexually transmitted diseases: 1 g at a time.

Dosage adjustment is not necessary in the elderly.

Dosage for the 125 mg film-coated tablet formulation is shown below.

Children up to 45 kg who can swallow a tablet (for the remaining group of children it is recommended to use the preparation in the form of a suspension):

Usually 10 mg / kg body weight is used once a day for 1 days; the exception is the dosage in the treatment of erythema migrans (the first symptom of Lyme disease): on the first day, 3 mg / kg body weight is used in a single dose, then 20 mg / kg body weight once a day for 10 consecutive days (from 1 to 4 . day).

For children weighing more than 45 kg, the adult dosing applies.

Dosage of the preparation in the form of granules for the preparation of an oral suspension:

Usually 10 mg / kg body weight is used once a day for 1 days; the exception is the dosage in the treatment of erythema migrans (the first symptom of Lyme disease): on the first day, 3 mg / kg body weight is used in a single dose, then 20 mg / kg body weight once a day for 10 consecutive days (from 1 to 4 . day).

For children weighing more than 45 kg, the adult dosage applies.

In case of mild to moderate renal impairment (creatinine clearance greater than 40 ml / min), no dosage adjustment is necessary. There are no data on the use of the preparation in people with a creatinine clearance lower than 40 ml / min and in such cases caution is recommended.

Dosage adjustment is not necessary in the presence of mild to moderate hepatic impairment. Do not use in patients with severe liver failure.

Oranex in the form of tablets is intended for oral use. 125 mg tablets should be taken once a day, at least 1 hour before or 1 hours after a meal. 2 mg tablets can be taken regardless of meals, once a day. Swallow the tablets whole (do not divide the tablets).

Oranex in the form of granules for suspension preparation is intended for oral use. The granules should be mixed with the appropriate amount of water indicated in the package leaflet (shake the bottle) in order to obtain a homogeneous suspension. The prepared suspension should be taken once a day, at least 1 hour before or 1 hours after a meal. Shake well before use. The suspension should be administered using the syringe or measuring spoon provided in the package. After administration, the suspension should be washed down with tea or juice so that the medicine does not remain in the mouth.

Oranex – Warning!

  1. Oranex should not be used in patients with severe liver disease, as there are no studies on its use in this patient group.
  2. Macrolide antibiotics (including the preparation) may rarely be associated with the occurrence of severe allergic reactions, including reactions that may be life-threatening, such as angioedema and anaphylactic reactions.
  3. Do not use the drug to heal infected burn wounds.
  4. It is recommended not to breastfeed while taking Oranexi for 2 days after stopping its use.
  5. Pregnant women should consult a doctor before use.
  6. The preparation should not be used simultaneously with ergot derivatives (e.g. ergotamine), because there is a risk of poisoning.

Oranex – side effects

Oranex may lead to the following side effects: visual disturbance, loss of consciousness, hypermobility, nervousness, fatigue and weakness (asthenia), tingling and numbness (paraesthesia), nausea, vomiting, diarrhea, abdominal pain, flatulence, digestive disorders, anorexia, constipation, glossitis, pseudomembranous enterocolitis (manifested by diarrhea), increased liver enzymes or increased bilirubin levels, rarely liver dysfunction (hepatitis, cholestatic jaundice, liver necrosis and liver failure).
