Mixing vaccines – what combination is most effective?
Recently, the medical journal “NEJM” (“The New England Journal of Medicine”) published the results of a study analyzing the effects of using different types of preparations for vaccination against COVID-19. This is called heterologous vaccination schedule. The study was conducted in 10 centers in the United States with 458 participants. 154 of them were boosted with Moderna’s vaccine, 153 with Pfizer’s vaccine, and 150 with Johnson & Johnson’s vaccine. One person did not hold a booster dose.
As it turned out? In all combinations of the preparations used, the increase in the amount of antibodies was significant. The titers of neutralizing antibodies against the SARS-CoV-2 virus increased by an average of 4 to 73-fold, and the titers of binding antibodies increased, on average, by 5 to 55-fold.

Homologous booster doses (from the same manufacturer as in the primary vaccination course) increased neutralizing antibody titers by 4 to 20 times, while in heterologous doses (different manufacturers), antibody titers increased from 6 to 73 times.
In the case of a specific immune response mediated by T lymphocytes, titers increased in all groups except for the one in which the homologous schedule of the J&J vaccine was administered. The best effects in this study occurred in those vaccinated first with J&J and then with the mRNA vaccine.

Mixing vaccines is safe and effective
In December, the European Medicines Agency (EMA) and the European Center for Disease Prevention and Control (ECDC) presented a joint position on the use of different types of vaccines, both in the primary vaccination regimen and in the booster dose.
Based on the analysis of multiple clinical trials supported by real world evidence, EMA announced that combining vaccines in a scheme, first vector vaccine (AstraZeneca, Johnson & Johnson), and then mRNA preparation (Pfizer, Moderna), leads to «Produce high levels of antibodies to the SARS-CoV-2 virus and give a higher T-cell response than using the same vaccine (homologous vaccination) in either a baseline or booster regimen«.

At the end of December, the position of the Polish Ministry of Health also changed, initially allowing vaccination only with the same preparation throughout the cycle. “Still the recommended basic regimen is the homologous regimen using one preparation, and in the case of mixed (heterologous) regimens, the following order is recommended: vector vaccine – mRNA vaccine. The reverse, mixed scheme can be implemented in the case of individual indications for such a procedure, said the Ministry of Health.
