Medics, as well as students of medical faculties from Poland and Ukraine, developed the Ukrainian version of medical interview forms. The link to the ready-to-download documents is provided below.
According to the Border Guard, over 500 people have already reached Poland. war refugees. Many of them need urgent medical attention. Others – very likely – will need it in the near future.
To facilitate contact with Ukrainian-speaking patients, a group of students prepared forms for a medical interview in Ukrainian.

– The language barrier is a huge problem, but thanks to the involvement of many people, medical students, it was possible to prepare forms in Ukrainian – explained Gilbert Kolbe, the press spokesman of the National Protesting and Strike Committee for Healthcare, an anesthesia nurse in an interview with Medexpress. – We are also working on forms in , because some refugees speak due to cultural and territorial conditions. We usually have interpreters at reception points to make it easier to communicate.
The forms are available on the website of the Polish Society of Family Medicine. They can be downloaded in PDF format.
The documents include, among others a pediatric, neurological, gynecological-obstetric, psychiatric interview form, as well as a data form for a prescription.
