Indications for use of the drug Ketrel
The active substance in Ketrel is quetiapine, a neuroleptic used in serious mental illnesses. Ketrel is used to treat schizophrenia and bipolar disorder (including: treating moderate to severe manic episodes in bipolar disorder, treating major depressive episodes in bipolar disorder, and preventing recurrence of bipolar disorder in patients who have responded to previous quetiapine treatment).
Dosage of Ketrel:
- Treatment of schizophrenia in adults – the total daily dose should be administered in 2 divided doses: 50 mg / day on day 1, 100 mg / day on day 2, 200 mg / day on day 3 and 300 mg / day on day 4 . After Day 4, 300-450 mg / day. Your doctor may advise you to adjust the dose in the range of 150-750 mg / day, depending on the clinical response and tolerability of the patient.
- For treatment of moderate to severe manic episodes in bipolar disorder in adults – the total daily dose should be administered in 2 divided doses: 100 mg / day on Day 1, 200 mg / day on Day 2, 300 mg / day on Day 3. on day and 400 mg / day on day 4. After day 4, the dose may be increased (according to your doctor’s recommendation) by a maximum of 200 mg / day, to a maximum of 800 mg / day on day 6. The maintenance dose is 200-800 mg / day depending on the patient’s reaction and tolerability, usually 400-800 mg / day.
- Treatment of major depressive episodes in adults with bipolar disorder; administered once daily in the evening: 50 mg / day on day 1, 100 mg / day on day 2, 200 mg / day on day 3 and 300 mg / day on day 4. The recommended dose is 300 mg / day; Depending on the patient’s response, the dose may be increased to 600 mg / day. No additional benefit was shown for the dose escalation to 600 mg / d compared to the 300 mg / d group for short term treatment.
- For the prevention of recurrence of bipolar disorder in patients with manic or depressive episodes responding to quetiapine treatment in adults it is recommended to continue the dosages determined individually for previous treatment; usually 300-800 mg / day in 2 divided doses; the lowest effective dose should be used.

Ketrel – warnings
- Elderly patients may need to titrate more slowly, and the daily dose lower, than that used in younger patients, depending on the clinical response and tolerability of the patient.
- The safety and efficacy of use in patients over 65 years of age with episodes of depression in the course of bipolar disorder have not been studied.
- There is no need to modify doses in patients with impaired renal function.
- In people with hepatic impairment initially 25 mg / day, the dose should be increased (under the supervision of a doctor) daily by 25-50 mg until the therapeutic dose is reached.
Ketrel and contraindications
Contraindication to the use of Ketrel is hypersensitivity to any component of the preparation, parallel use of: CYP3A4 inhibitors of cytochrome P450, such as protease inhibitors used in the treatment of HIV, azole antifungals, erythromycin, clarithromycin and nefazodone.
