Captopril Jelfa is a drug from the group of angiotensin converting enzyme inhibitors, which, like all preparations from this group, inhibits the formation of angiotensin II, a substance that causes vasoconstriction and stimulates the release of aldosterone.

Captopril Jelfa – indications for use
Captopril Jelfa is taken in:
- treatment of hypertension,
- chronic heart failure (weakened contractions of the ventricles),
- diabetic nephropathy with proteinuria in patients with type 1 diabetes,
- in a situation after a heart attack (treatment about 4 weeks),
- treatment of all patients in a stable clinical condition (24 hours after MI),
- long-term prevention of heart failure,
- treatment of people with left ventricular dysfunction (ejection fraction not greater than 40%).
Captopril Jelfa and contraindications to use
Contraindication to taking Captopril Jelfa is:
- hypersensitivity to any of the ingredients or to any other angiotensin converting enzyme (ACE) inhibitor,
- a history of angioedema or congenital (hereditary) angioedema
- significant bilateral stenosis of the renal arteries or unilateral in the case of one active kidney,
- simultaneous use of extracorporeal circulation procedures (hemodialysis, haemofiltration, low-density lipoprotein apheresis, LDL), during which the blood is exposed to contact with electrically negatively charged surfaces,
- Captopril Jelfa is contraindicated in the second and third trimesters of pregnancy and during breastfeeding,
- It is not recommended to use the preparation in the first trimester of pregnancy.
Captopril Jelfa – dosage
Captopril Jelfa is in the form of tablets and is intended for oral use. The drug can be taken regardless of meals. Do not exceed the recommended doses, as it may harm your life or health.
Dosage of Captopril Jelfa in adults with:
- high blood pressure
- The maximum dose of 150 mg per day. In the case of combination treatment with other antihypertensive drugs, e.g. thiazide diuretics, the doctor may recommend captopril to be taken once a day.
- with a strong activation of the renin-angiotensin-aldosterone system (e.g. renovascular hypertension, decompensated heart failure, decreased intravascular volume)
- heart failure

Treatment of nephropathy in patients with type 1 diabetes:
- 75-100 mg daily in divided doses. If necessary, your doctor may add other antihypertensive medications to your treatment.
Dosage of Captopril Jelfa in patients with impaired renal function
- in this case, individual dosage adjustment is necessary; the doctor determines the appropriate dose in individual cases. If the creatinine clearance is less than 40 ml / min but greater than 20 ml / min, the daily starting dose is 25 mg and the maximum daily dose is 100 mg. If the creatinine clearance is less than 20 ml / min but greater than 10 ml / min, the starting daily dose is 12,5 mg and the maximum daily dose is 75 mg. In severe renal failure, if the creatinine clearance is less than 10 ml / min, the starting dose is 6,25 mg daily and the maximum dose is 37,5 mg daily. In these people, treatment can only be carried out under strict medical supervision. If it is necessary to use diuretics in parallel, in patients with severe renal insufficiency, it is recommended to use loop diuretics (e.g. furosemide) instead of thiazide diuretics.
Dosage of Captopril Jelfa in elderly patients should be determined by the doctor, because in these patients there is an increased risk of side effects, especially in the case of impaired renal function or other organs. Your doctor will work out a recommended starting dose and then gradually increase it until you achieve the desired clinical response. There are no complete data on the safety and efficacy of Captopril Jelfa in children and adolescents. For this reason, treatment should only be initiated in this age group under close medical supervision. The dose and the interval between doses will be determined by the doctor in each case individually.
