Hello, I’m 34 years old and often when I was young problems with sinusitis. However, I remember that severe pains in the temples and around the bridge of the nose appeared along with a thick runny nose. Now I have similar headaches in and around the bridge of my nose, but none of it nasal discharge. Can you have sinusitis without a runny nose? How do I know if my symptoms are really sinusitis and not anything else – more serious and dangerous? Should I have a head CT scan to be sure?
Sinusitis I used to have recurring recurrences, but for a long time the symptoms had resolved. I also suffer from migraines, and I don’t know if one could be related to the other. My mom says it’s probably the sinuses again and that I should see a doctor right away to act until the pain is so bad. Headache now also occurs when I bend down for something. Then I have a feeling of scotoma in front of my eyes.
Please clarify my doubts whether it is possible to have sinusitis without a runny nose. Earlier, it seemed impossible to me, because blocked sinuses and discharge in the sinuses causes them to become inflamed and only then does pain appear. But now it looks completely different in my case, and I’m a little scared. Regards.

Madam, sinusitis can run without a runny nose, especially if we are dealing with chronic sinusitiswe are talking about when symptoms last for at least 12 weeks. Sinusitis can be manifested by local pain, pressure, or constant feeling stuffy nose. Runny nose may or may not be present. The causes of sinusitis may be different. First of all, you should take into account bacterial infections, but also allergies, airway obstruction, insufficiency of the ciliary apparatus, air pollution or tonsillitis.
The treatment uses anti-inflammatory drugs, including ibuprofen or ketoprofen orally, as well as topical medications in the form of drops – steroids are mainly used in this form. If a bacterial etiology is suspected, antibiotic therapy is required. In your case, I suggest you do CT scan of the sinuses paranasal due to the fact that the symptoms have been cyclical for many years. There is definitely no point in taking a regular sinus X-ray as it is not relevant for the diagnosis of sinusitis.
Thanks to CT, it is possible to visualize the inside of the sinuses and possible pathological changes, including the presence of polyps or thickening of the mucosa. With the test result, you should go to the ENT specialist. When it comes to migraines, one does not exclude the other. So the symptoms may be due to possible sinusitis or migraine itself, although without further research it cannot be determined specifically.
