How PIROGOV.AI solves the problem of queues to the otolaryngologist with the help of AI

PIROGOV.AI is a system for establishing an expert diagnosis of ENT diseases and supporting medical decision-making based on endoscopic photo and video images

Task

Create a solution that would allow general practitioners (general practitioners, pediatricians, paramedics) to independently diagnose ear, nose and throat diseases with a high degree of accuracy, prescribe treatment themselves, and only in more complex cases, with appropriate symptoms and indications, redirect patients to an ENT specialist.

Problem

According to the Ministry of Health, every year more than 6% of the country’s population visits an ENT doctor only with ear diseases, which means that 9 million people a year need an otolaryngologist consultation.

With such a heavy workload for ENT specialists, it is not always easy to make an appointment with them in the conditions of municipal medicine. The queue for recording can be up to two weeks, and the quality of treatment sometimes leaves much to be desired. International medical practice shows that otolaryngologists are wrong in every fourth diagnosis, and general practitioners (general practitioners, pediatricians, paramedics) – in every second. This leads to complications or to a situation where the patient can be treated with different antibiotic regimens for several months as a result of an error.

This leads to a significant increase in the period of treatment of diseases, a decrease in labor productivity and an increase in payments for temporary disability, as well as reputational risks for medical institutions due to medical errors.

PIROGOV.AI has created a solution that allows municipal medicine to introduce full control over the correctness of the diagnosis at outpatient appointments. The solution will help commercial medicine not only significantly improve the quality of diagnosis, but also enter the market for the provision of ENT services by general practitioners, as well as increase business margins through the diagnosis and treatment of complex ENT diseases.

Solution

The PIROGOV.AI system makes it possible to increase the accuracy of making a correct diagnosis up to 95% or more and is used in various interaction scenarios: patient-doctor or doctor-doctor, including using telemedicine technologies. The solution is based on artificial intelligence technologies and allows accurate diagnosis of ENT diseases based on the analysis of endoscopic photo and video images. The hardware-software complex consists of a portable endoscope and software that can be installed as a mobile application on a smartphone. Thus, the primary care physician can diagnose the patient both in the clinic and at home.

The primary care doctor uploads an endoscopic image or video to the PIROGOV.AI system and receives a preliminary diagnosis, indicating the likelihood of a particular disease, in the form of a consultation opinion of the recognition (diagnosis) results performed using a trained neural network. With the help of telemedicine technologies, a primary care doctor in complex cases can remotely connect an otolaryngologist online, get additional advice, clarify the diagnosis and prescribe the right treatment. The PIROGOV.AI solution is relevant both in the case of a high workload of ENT doctors in a particular region, and in case of their absence within reach, for remote regions.

Prospects

In the future, all primary care physicians can be equipped with the solution: general practitioners, general practitioners, pediatricians, paramedics, of whom there are more than 250 thousand in our country. This will significantly improve the quality of diagnosis and provision of medical care to patients, reduce treatment time and losses from temporary disability .

The results

Currently, the project is being implemented in four private clinics in our country. The results of testing the system showed a high accuracy of diagnosis – over 95% for the ten most common nosologies of ear diseases in outpatient practice. In 2020, as part of the BioMedTech track of the Moscow accelerator program, in which the PIROGOV.AI project became a participant, it was decided to introduce the system into the EMIAS service (Unified Medical Information and Analytical System. — Trends).
