Fluconazole Redibag 2 mg/ml

Fluconazole is an antifungal medicine from the group of triazole derivatives for systemic use. This preparation is indicated, among others, in the treatment of deep infections and peritonitis, infections of the mouth, esophagus and as a prophylactic treatment of severe yeast infections in patients after bone marrow transplantation. The drug is intended for use by both adults and children. Fluconazole is supplied as a prescription infusion solution.

Indications for the use of Fluconazole

Treatment of the following infections in adults:

  1. systemic candidiasis, including disseminated, deep infections and peritonitis,
  2. severe yeast infections of the mucous membranes of the mouth, throat and esophagus,
  3. non-invasive bronchial and pulmonary candidiasis when oral treatment is not possible, cryptococcal meningitis.
  4. Prevention of severe yeast infections in patients with weakened immunity after bone marrow transplantation.
  5. Treatment of the following infections in children and adolescents: systemic candidiasis, including disseminated deep infections and peritonitis, severe oral, pharyngeal and esophageal yeast infections, non-invasive bronchial and lung candidiasis when oral treatment is not possible.

Children and youth:

treatment of mycoses caused by Candida and other susceptible species of yeasts, especially:

  1. systemic candidiasis (including disseminated deep infections and peritonitis),
  2. severe mucosal candidiasis (including oropharyngeal candidiasis, oesophageal candidiasis and non-invasive bronchopulmonary candidiasis) when oral therapy is not available.

Contraindications to the use of Fluconazole

Contraindications to taking Fluconazole are:

  1. allergy to any ingredient of the preparation or other preparations from the group of azole derivatives,
  2. simultaneous intake of astemizole, cisapride, pimozide, quinidine and terfenadine and other drugs that prolong the QT interval on the ECG.

Dosage Fluconazole

Adults:

  1. Systemic candidiasis (blood yeast infections, disseminated candidiasis and other forms of invasive yeast infection): the starting dose is 400-800 mg daily, then 200-400 mg daily. The duration of treatment depends on the clinical response. If yeast is present in the urine, the recommended dose is 100 mg daily for 14-30 days.
  2. Severe oropharyngeal mucosa candidiasis: 100 mg once daily for 1-7 days.
  3. Severe candidiasis of other mucous membranes (except for genital candidiasis), if oral administration is impossible: 100 mg once a day for 1-14 days.
  4. Cryptococcal meningitis (initial treatment): The starting dose is 400 mg per day, then 200-400 mg per day. Duration of treatment depends on clinical response, usually at least 6-8 weeks.
  5. To prevent recurrence of cryptococcal meningitis in AIDS patients: after initial treatment with the full dose, the usual dose is 200 mg daily; this dose can be taken for a long time.
  6. Prevention of severe yeast infections in immunocompromised patients after bone marrow transplantation: 400 mg daily. Your doctor will start treatment a few days before the expected onset of neutropenia and will advise you to continue treatment for 7 days after your blood reaches a neutrophil count of more than 1000.

Children and youth:

  1. Due to limited clinical data, the preparation is not recommended in children and adolescents up to 16 years of age. The preparation in children and adolescents up to 16 years of age can be used only if the doctor deems it absolutely necessary.

Newborns 4 weeks old and younger:

  1. Newborns excrete fluconazole more slowly, it is necessary to increase the time intervals between doses. Data in neonates are limited. In particular, there are no data on the use of the preparation in newborns with impaired renal function.
  2. In the first 2 weeks of life, the same dose (6-12 mg / kg body weight) is used as in older infants, but given every 72 hours. At 3 and 4 weeks of age, the same dose is given every 48 hours.
  3. In newborns in the first 2 weeks of life, the dose should not exceed 12 mg / kg body weight every 72 hours, and in newborns in the third and fourth week of life, the dose should not exceed 3 mg / kg body weight every 4 hours.

Children and infants after 4 weeks of age:

  1. Systemic candidiasis: 6-12 mg / kg body weight daily.
  2. Cryptococcal infections: 6-12 mg / kg body weight daily.
  3. Mucosal candidiasis: 3 mg / kg body weight once a day. On day 1, your doctor may recommend a single dose of 1 mg / kg body weight per day.
  4. The maximum dose in children is 400 mg daily.

Special groups of patients:

  1. Dosage adjustment is not necessary in elderly patients with normal renal function.
  2. In people with impaired renal function, if the creatinine clearance is greater than 50 ml / min, no dosage adjustment is necessary. If the creatinine clearance is less than 50 ml / min, dosage adjustment by the doctor is necessary (except for the treatment of vaginal candidiasis). Usually, your doctor will prescribe the standard dose on the first day and then reduce the dose by half. In patients undergoing dialysis, the recommended dose should be taken after each dialysis.

Fluconazole must only be given as an infusion into a vein.

Fluconazole – warnings

  1. Patients with severe cryptococcal meningitis should use the drug with caution.
  2. Test abnormalities have been seen in people with: severe underlying diseases such as AIDS and cancer during treatment with fluconazole
  3. hematology, liver, kidney and other biochemical parameters.
  4. Patients who develop liver function test abnormalities while undergoing treatment with fluconazole should be monitored for more serious liver damage.
  5. Rarely, exfoliative skin reactions such as Stevens-Johnson syndrome and toxic epidermal necrolysis have been reported in patients during treatment with fluconazole.
  6. AIDS patients are at greater risk of developing severe skin reactions to many medicinal products.
  7. The drug passes into breast milk. Breastfeeding after multiple or high doses of fluconazole is not recommended.
  8. During the first trimester of pregnancy, the preparation Fluconazole has no harmful effects on the fetus.
