Femur fractures – symptoms. What is the treatment of a shaft and peripheral femoral fracture?

They may affect the upper femur (neck fractures and trochanteric fractures) as well as the shaft and peripheral end of this bone.

Upper femoral fractures (neck and trochanteric fractures)

They usually occur in the elderly, more often in women, as a result of osteoporosis. The most common cause is a minor trauma, such as stumbling on stairs, a carpet, stepping on the foot harder, falling to the side. There is pain in the hip or knee on the side of the fracture, and trying to move causes pain. The limb is twisted outward, often shortened.

First aid. To immobilize a limb, it is best to use the second healthy limb as a splint. Both limbs are tied together, including the feet. For this you need two sticks or two slats of different lengths. The shorter one should reach from the groin to the heel, the longer – from the armpit to the heel. Both of these items are tightly bandaged with each other, additionally bandaging both feet.

Treatment. Surgical, the aim of which is to restore the patient’s fitness to the level he had before the fracture. The conservative treatment used in the past, such as extracts, plaster dressings, was abandoned due to the large number of fatal complications caused by prolonged bed rest. Surgical treatment, regardless of the age of the patients, allows to avoid fatal complications and quickly restore the functionality of the broken limb. Already in the first day after surgery, the patient can be started from the bed.

Femoral shaft fractures

They arise as a result of a very strong:

• direct injury, as multi-fracture fracture,

• indirect injury, as oblique spiral fracture.

Fracture Symptoms: pain, shortening of a limb, abnormal thigh bend, sometimes a thickening of the thigh and painful movement at the fracture site.

Treatment. Immobilization should cover the hip joint, thigh and lower leg and be close to immobilization in the case of a neck fracture. With the exception of fractures in children, surgical treatment should be applied. Currently, the most frequently used method of surgical treatment is the intramedullary nail.

Fractures of the peripheral end of the femur

They are severe fractures, most often occurring during traffic accidents. The action of the muscles attached here leads to the displacement of the fragments. Stands out extra-articular fractures i intra-articular. They may be accompanied by damage to the ligaments of the knee joint, the femoral or popliteal artery, and damage to the peroneal nerve. These fractures require surgical treatment.
