Disorders of the statics of the genital organs may take the form of, for example, faulty positioning. Then they are a consequence of the failure of the pelvic floor and the suspension apparatus, which fixes the structures of the smaller pelvis and / or the abdominal wall. This leads to a change in the direction of pressure in the minor pelvis from the cephalic to the caudal.
Disorders of statics of the reproductive organ
Vertical displacements give rise to varying degrees vaginal hernia, which is the result of the loosening of the structures that make up the pelvic diaphragm. A vaginal hernia is always accompanied by too much mobility of the uterus. The relaxation of the pelvic diaphragm in virtually all cases is the result of tearing or stretching of the pelvic diaphragm tissues during labor. The consequence of this is a slow weakening of the suspension apparatus, to which predispose to the long-term effect of gravity in an upright position, and sudden increases in pressure in the abdominal cavity, e.g. when lifting, coughing or sneezing. An additional age-related cause is the weakening of connective tissue resulting from a decrease in the concentration of sex hormones, which is especially observed after the menopause. The individually variable value of connective tissue also plays a significant role.
Classification of disorders of the statics of the sexual organ:
- lowering the utero-rectal cavity (enterocele),
- lowering the back wall of the vagina (rectocele),
- lowering the uterus,
- lowering the anterior vaginal wall (cystocele),
- urethral diverticulum.
We distinguish three degrees in each of the above-mentioned types. Stage III is characterized by complete loss of the proper structure, e.g. the anterior vaginal wall or the uterus.
Disorders of statics of the reproductive organ – symptoms
Small and larger vaginal decreases can sometimes run without characteristic symptoms. Often, however, women complain of a prolapse of the uterus and a feeling of gravity in the lower abdomen. Symptoms are especially severe with physical activity, exertion, prolonged standing, coughing, sneezing or running. The decline of the reproductive organ is often accompanied by a symptom such as urinary incontinence. Moreover, frequent urination and an increased predisposition to urinary tract infections may occur. In turn, complete prolapse of the reproductive organ may cause urinary retention or difficulty passing urine. It is also not uncommon to see pressure ulcers. Less common symptoms of disorders of the statics of the reproductive organ are:
- constipation
- vaginal discharge,
- back pain,
- inguinal pain.
Diagnostics of disorders of the statics of the reproductive organ
In the diagnosis of disorders of the statics of the reproductive organ, it is very important to examine the patient, who should be lying down during the examination. During the examination, the doctor asks the patient to try to perform a pressure reflex. Another examination is a standing examination, during which the specialist looks at the vaginal opening at the same time. In addition, the levator ani muscles and the tension of the anterior abdominal wall are examined.
In the case of vaginal depression or simultaneous lowering of the vagina and uterus, it is necessary to conduct a diagnosis for urinary incontinence.
How to treat disorders of the statics of the reproductive organ?
Conservative measures include:
- exercises for the smaller pelvis (especially recommended for young women),
- treatment with estrogens that do not affect the uterine mucosa, but improve the tone and vascularization of supporting tissues,
- vaginal inserts – recommended especially for women who, for various reasons, cannot undergo surgery.
Occasionally, your doctor may also recommend pessary treatment. You can now buy pessaries tailored to your needs on Medonet Market. Another method of treating disorders of the statics of the reproductive organ is surgery. There are a number of surgical methods available to suit the type and degree of static disorders as well as comorbidities and the age of the woman. In young women, reconstruction of the pelvic floor is usually performed. On the other hand, if it is necessary to preserve the uterus, the procedure of uterine lifting (menchester surgery) or other plastic surgeries of the vagina and perineum, with or without suspension of the uterus, is performed.
In pubertal women, the uterus is often removed with simultaneous plastic surgery of the vagina and pelvic floor muscles. In older women, partial vaginal stitching can be performed.
Prevention
In order to avoid disturbances in the statics of the reproductive organ, prophylaxis is very important, which consists in preventing:
- overloads,
- chronic cough,
- birth injuries
- constipation and the fight against obesity.
Well-chosen gymnastic exercises also play an important role.
