DicloDuo is a preparation used in rheumatology and orthopedics, traumatology of the musculoskeletal system for the treatment of rheumatic diseases and post-traumatic inflammation of the limbs. DicloDuo has analgesic, anti-inflammatory and antipyretic properties. It is issued by prescription.

DicloDuo – action

DicloDuo is a non-steroidal anti-inflammatory drug that is used to treat a variety of joint diseases. You can get this drug from a pharmacy with a prescription. DicloDuo is in the form of modified-release capsules under the name:

  1. DicloDuo 75 mg in packages of 30 capsules;
  2. DicloDuo Combi 75 mg + 20 mg in packs of 30 capsules.

The active substance in DicloDuo is diclofenac sodium. Each capsule contains 25 mg of gastro-resistant pellets and 50 mg of diclofenac as sustained release pellets. In addition, the Combi version is also omeprazole.

Diclofenac is an aminophenylacetic acid derivative that has anti-inflammatory, antipyretic and analgesic effects. In addition, it has properties that reduce platelet aggregation. This substance has no effect on glucose metabolism.

DicloDuo works by stopping the synthesis of prostaglandin cyclooxygenasewhich is causing inflammation. When swallowed, the drug is almost completely absorbed in the gastrointestinal tract, and the modified-action capsules quickly release 1/3 of the dose, and the rest is gradually released during the day.

DicloDuo – composition

The DicloDuo lineup includes:

  1. DicloDuo – 75 mg of diclofenac sodium divided into 25 mg in gastro-resistant pellets and 50 mg in long-acting pellets, and additionally the enteric pellets contain microcrystalline cellulose, povidone k25, anhydrous colloidal silica, methacrylic acid and ethyl acrylate copolymer with 0,28 mg sodium hydroxide, propylene glycol and talc. In turn, the long-acting pellets contain microcrystalline cellulose, povidone k25, anhydrous colloidal silica, copolymer (type B) of ammonium methacrylate – Eudragit RS 100, copolymer (type A) of ammonium methacrylate – Eudragit RL 100, triethyl citrate and talc. The coating consists of titanium dioxide (E 171), indigo carmine (E 132), purified water, gelatin, and the ink contains shellac, propylene glycol and titanium dioxide (E 171);
  2. DicloDuo Combi – the hard gel capsules contain 75 mg of diclofenac sodium and 20 mg of omeprazole. Additionally, each capsule contains microcrystalline cellulose, povidone K25, anhydrous colloidal silica, methacrylic acid and ethyl acrylate type A copolymer neutralized (6 mol%) with sodium hydroxide, propylene glycol, A type ammonio methacrylate copolymer, B type ammoniomethacrylate copolymer, mannitol, magnesium, hydroxypropyl cellulose, sodium lauryl sulfate, hypromellose, methacrylic acid and ethyl acrylate copolymer, polysorbate 80, triethyl citrate and talc. The DicloDuo Combi casing contains titanium dioxide (E 171), red iron oxide (E 172), yellow iron oxide (E 172) and gelatin.

DicloDuo – indications

DicloDuo is used for the symptomatic treatment of:

  1. inflammatory rheumatic diseases (rheumatoid arthritis, osteoarthritis, ankylosing spondylitis),
  2. post-traumatic and rheumatic inflammation of tendons, ligaments and periarticular soft tissues,
  3. analgesic after surgical procedures,
  4. in painful menstruation.

In the diagnosis of rheumatoid arthritis, a package of blood tests for rheumatoid arthritis may be useful.

DicloDuo – contraindications

Contraindications to the use of DicloDuo are:

  1. hypersensitivity to any component of the preparation or to other non-steroidal anti-inflammatory drugs;
  2. active gastric and / or duodenal ulcer disease;
  3. severe renal failure;
  4. liver or heart;
  5. XNUMXrd trimester of pregnancy;
  6. history of reactions to acetylsalicylic acid or other NSAIDs such as bronchospasm, rhinitis, urticaria, asthma;
  7. history of gastrointestinal bleeding or perforation after taking NSAIDs;
  8. disorders of hematopoiesis for unknown reasons;
  9. bleeding from cerebral vessels or other active bleeding;
  10. congestive heart failure;
  11. ischemic heart disease;
  12. peripheral vascular disease and / or cerebrovascular disease;
  13. hepatic and / or renal failure;
  14. the period of breastfeeding;
  15. Age: Diclo Duou is not recommended for use in children and adolescents.

With DicloDuo Combi, concomitant use of nelfinavir is not recommended due to the omeprazole content. Hypersensitivity to benzimidazoles is also a contraindication.

Diclo Duo – dosage

The high content of diclofenac in DicloDuo makes the drug suitable for adults. The dosage is always determined by the doctor who determines it based on the reason for the treatment and adjusts the dose to the patient and his situation. DicloDuo capsules is served after a meal. Swallow them whole and drink plenty of water.

The dosage is as follows:

  1. DicloDuo – it is recommended to take 1 drug capsule daily (75 mg). If necessary, the doctor will decide to increase the dose to 2 capsules a day. If required by the patient’s condition, it is allowed to use 2 DicloDuo capsules in one daily dose. The doctor decides when to use the drug;
  2. DicloDuo Combi – it is recommended to use 1 capsule once a day. Higher doses of the drug cannot be used as this increases the exposure to omeprazole. Treatment continues until the expected results appear. The patient’s condition should be assessed regularly during treatment. If the above-mentioned dose is ineffective, the doctor decides to change the treatment.

Particular care should be taken by:

  1. people with kidney problems – this also applies to people with mild or moderate impairment. This does not require dose adjustment, but renal function should be monitored in this patient group. If the patient has severe renal impairment, it is a contraindication to taking DicloDuo;
  2. people with liver disorders – during treatment, caution should be exercised already in the case of people with mild or moderate hepatic impairment. It is advisable to monitor organ function, but it is not necessary to change the dose amount. DicloDuo is forbidden if the patient develops severe liver dysfunction;
  3. the elderly – no dose adjustment is required, but care should be taken. People with low weight should use the lowest dose possible for the shortest possible time. In addition, constant monitoring is necessary for the appearance of gastrointestinal bleeding;

DicloDuo should not be given to children and adolescents under 18 years of age.

DicloDuo – overdose

In the event of an overdose of DicloDuo, contact your doctor as soon as possible. If a child swallows the medicine, seek medical attention immediately. Symptoms of DicloDuo overdose they can be very different and can include:

  1. vomiting;
  2. diarrhea;
  3. gastrointestinal bleeding;
  4. dizziness;
  5. Tinnitus;
  6. seizures;

DicloDuo – side effects

DicloDuo, like any other drug, may cause certain side effects. However, it should be remembered that they do not appear in every patient. In addition, the most common benefits of using DicloDuo outweigh the possible risks associated with its use.

The risk of side effects can be minimized by using the drug for the shortest possible time.

Side effects may be serious and in this situation require immediate consultation with a doctor. These symptoms include:

  1. allergic reactions including difficulty breathing, wheezing, swelling of the face, lips, mouth, hands or fingers, skin rash, itching, bruising, peeling skin, painful red skin patches or blisters;
  2. chest pains – a possible symptom of Kounis syndrome;
  3. stomach pain or other gastrointestinal symptoms;
  4. cramps and tenderness around the abdomen – these symptoms are felt immediately after swallowing the capsule, followed by rectal bleeding or bloody diarrhea during the day;
  5. heartburn and indigestion;
  6. blood in the stool;
  7. bleeding in the stomach or intestines such as tarry stools
  8. vomiting blood or dark particles that look like coffee grounds.

  1. severe allergic reactions including fainting, rash, wheezing, swelling or difficulty swallowing;
  2. yellow skin, dark urine or tiredness – a symptom of liver problems;
  3. reddening of the skin with painful spots, blistering or peeling skin – signs of Stevens-Johnson syndrome or toxic epidermal necrolysis.

Other possible side effects of using DicloDuo include:

  1. common: headache, dizziness, loss of appetite, nausea, diarrhea, vomiting, flatulence, heartburn, indigestion and other gastrointestinal symptoms, rash and disturbances in blood tests for liver function;
  2. rare: allergic reactions, difficulty swallowing, drowsiness, fatigue, blood in the stools, bleeding from the stomach or intestines, vomiting blood, stomach or duodenal ulcer, perforation of the gastrointestinal tract, liver disorders, rash, hives, jaundice;
  3. very rare: incl. depression, confusion, anxiety, mood changes, irritability, memory impairment, hallucinations, confusion, insomnia, nightmares, tingling, pins and needles, stiff neck, diplopia, tinnitus, constipation, double vision, esophageal disorders, photosensitivity, anemia etc. .

The active substance is DicloDuo may increase the risk of a heart attack or stroke to some extent. Other possible side effects of NSAIDs include edema as a consequence of fluid build-up, increase in blood pressure, chest pain, heart failure and palpitations.

With DicloDuo Combi, due to the presence of omeprazole, the following may appear:

  1. common: headache, vomiting, nausea, diarrhea, constipation, stomach pain, flatulence or gastric polyps;
  2. uncommon: insomnia, dizziness, sleepiness, stinging and numbness, swelling of the feet and ankles, hives and itchy skin, feeling unwell, changes in blood tests related to liver function, fractures of the wrist, hip or spine;
  3. rare: allergic reactions, sudden difficulty breathing or shortness of breath, agitation, confusion, depression, altered taste, vision, dry mouth, inflammation inside the mouth, thrush, liver disorders, jaundice, hair loss, sun rash, pain muscles and joints, severe kidney disorders, severe sweating, blood count disturbances;
  4. very rare: hallucinations, aggression, male gynecomastia, erythema multiforme, toxic epidermal necrolysis, Stevens-Johnson syndrome, changes in blood cell counts, severe liver problems leading to liver failure and inflammation of the brain.

In addition, inflammation of the intestine may also develop, with diarrhea and rash, accompanied by joint pain.

Remember!

People taking DicloDuo Combi for more than 3 months may experience a drop in blood magnesium levels. It manifests itself with muscle cramps, disorientation, fatigue, dizziness, convulsions, faster heart beat. These symptoms are an indication for consultation with a doctor.

In addition, omeprazole may lower your immunity. Therefore, if you develop symptoms of an infection, such as fever, pain in the neck, mouth, problems with urination, or deterioration of the general condition of the body, you should seek medical help quickly.

DicloDuo – alcohol

Do not combine DicloDuo with alcohol. There may be moderate interactions. However, alcohol may increase the side effects of the drug, and thus, for example, increase the risk of gastrointestinal bleeding. In addition, people who abuse alcohol are at risk of developing acute renal failure.

DicloDuo – pregnancy and breastfeeding

DicloDuo should not be used in women in the third trimester of pregnancy, and in the first and second trimesters it can be administered if absolutely necessary.

DicloDuo used during pregnancy may increase the risk of miscarriage or the baby may develop heart defects and gastroschisis. The duration of treatment and dose level increase the risk. If a woman needs to take DicloDuo, these should be the lowest dose possible and given for the shortest possible time.

In the third trimester of pregnancy, DicloDuo has a negative effect on both the baby and the mother. The drug may stop uterine contractions and prolong labor, and in the fetus it may lead to pulmonary hypertension with premature closure of the ductus arteriosus, or renal dysfunction leading to renal failure with oligohydramnios. In both mother and child, DicloDuo may lead to longer bleeding or anti-aggregation effects.

It should also be remembered that diclofenac passes into breast milk in small amounts, therefore it is not recommended to take the drug while breastfeeding.
