Acute, if not treated or neglected, glaucoma may turn into chronic glaucoma. The attacks of the disease are repeated or the intraocular pressure is constantly elevated, which leads to the atrophy of the optic nerve and irreversible loss of vision.

The disease usually develops in both eyes, but not simultaneously; it is more common in women and in people with hyperopia.

Mental factors play a large role in the occurrence of glaucoma attacks, especially severe anxiety and worry; they cause an increase in intraocular pressure. An attack of glaucoma may also cause pupil dilation, e.g. as a result of staying in dark rooms, using eye drops or taking medications that widen the pupil.

In chronic glaucoma, conservative treatment is usually ineffective and surgery is required.
