Pregnancy: why are we bedridden?

It is the fear of all future mothers: to be bedridden. Clearly, having to spend the rest of her pregnancy near her bed or sofa. But rest assured, we are not prescribing forced rest for any reason. The main indication for bed rest is the threat of premature labor (PAD). It is defined by a changes in the cervix before 8 months of pregnancy, associated with regular and painful uterine contractions. Normally, the cervix is ​​extremely strong and effective in sustaining pregnancy until term. Therefore, there is no contraindication to walking or playing sports while pregnant. On the other hand, if the future mother has a contractile uterus and her cervix begins to change, excessive movement can make the situation worse. To reduce uterine contractions, block the opening of the cervix and thus allow the pregnancy to continue as long as possible, the doctor then orders strict rest.

Note: there are different levels in bed rest. The setting to rest is indeed graduated according to the risk of preterm birth : from a few hours a day at home to hospitalization in a specialized maternity ward if the cervix is ​​very open.

A change in the cervix

The modification of the cervix during pregnancy is the first indication for bed rest. There are two exams to detect this anomaly. With vaginal examination, the gynecologist assesses the position, consistency, length and closed nature of the cervix. It is an interesting examination but it has the drawback of being subjective. Hence the interest of practicing a endovaginal cervical ultrasound. This exam allows you to know precisely the length of the collar. In 2010, the Haute Autorité de santé reiterated the value of this medical act. Generally, if the cervix is ​​less than 25 mm, the risk of preterm delivery is increased and hospitalization may be necessary.

Premature rupture of the water bag

Normally, water is lost during or shortly before childbirth. But it can happen that this loss occurs much earlier. Before 7 months of pregnancy, we speak of premature rupture of the water bag. In this case, there is a indication to be bedridden. Indeed, once part of the amniotic fluid escapes, there is a risk of infection because the baby is no longer in a sterile environment. Not only can the infection affect the development of the fetus, it can also cause contractions and induce labor. It is estimated that nearly 40% of premature deliveries are due to an anticipated rupture of membranes.

Uterine malformations

2-4% of women have congenital malformations of the uterus, for example a septate uterus, bicorne (two cavities) or unicorne (a half). The consequence ? The baby develops in a uterus that is not its normal size and therefore quickly becomes cramped. The first contractions, instead of appearing at term, will occur in the middle of the pregnancy, causing an early onset of labor. With plenty of rest it is possible to delay delivery for several weeks.

A woman who is bedridden during her first pregnancy is not necessarily so for her second child.

We are rarely bedridden before 3 months of pregnancy.

For multiple pregnancies: rest is essential. The pregnant woman usually stops working during the 5th month. This does not mean that she is necessarily bedridden.
